目的：探讨胃肠胰神经内分泌肿瘤的临床特点。方法回顾性分析均经病理证实的胃肠胰神经内分泌肿瘤26例患者的临床资料。结果26例患者中发生在直肠最多，占50．0％（13/26）；Syn和CgA阳性率分别为92.31％和76．92％，Ki-67的表达阳性率为34．62％，Syn与CgA联合检测的阳性率为100％；Ki-67的表达与患者的年龄、性别无明显相关（P＞0．05），而与肿瘤的大小、有无转移、分期显著相关（P＜0．01）；肿瘤病理分类、分级及远处转移均与生存相关（P＜0．01）。结论直肠是胃肠胰神经内分泌肿瘤的好发部位，临床表现缺少特异性，内镜和影像学检查是重要的诊断手段，确诊依靠病理，其预后与肿瘤的大小、分类、分级、转移情况等有关。 Syn与CgA联用诊断的敏感性较高，Ki-67是有效的判断预后的指标。
목적：탐토위장이신경내분비종류적림상특점。방법회고성분석균경병리증실적위장이신경내분비종류26례환자적림상자료。결과26례환자중발생재직장최다，점50．0％（13/26）；Syn화CgA양성솔분별위92.31％화76．92％，Ki-67적표체양성솔위34．62％，Syn여CgA연합검측적양성솔위100％；Ki-67적표체여환자적년령、성별무명현상관（P＞0．05），이여종류적대소、유무전이、분기현저상관（P＜0．01）；종류병리분류、분급급원처전이균여생존상관（P＜0．01）。결론직장시위장이신경내분비종류적호발부위，림상표현결소특이성，내경화영상학검사시중요적진단수단，학진의고병리，기예후여종류적대소、분류、분급、전이정황등유관。 Syn여CgA련용진단적민감성교고，Ki-67시유효적판단예후적지표。
Objective To understand the pathological characteristics of gastrointestinal pancreatic neuroendocrine neoplasms ( GEP-NENs ) .Method The analysis was conducted mainly by reviewing the clinical data of 26 patients of GEP-NENs.Findings Of the 26 cases of GEP-NENs, 13 cases were found to have the disease originating in the rectum . Syn and CgA positive rates were 92.31% and76.92%.The positive expression rate of Ki-67 was 34.62%.The positive rate of combined detection of Syn and CgA was 100%.No significant correlation was found between age , gender and the expression of Ki-67 (P>0.05), while tumor size, metastasis, stage were significantly correlated (P<0.01).Conclusion Rectum is the GEP-NENs predilection site, bearing no specific clinical manifestations.Endoscopy and radiographicexamination are two practical methods currently employed in diagnosing the disease .However, critical diagnosis depends onpathological study.The prognosis can be affected by tumor size , histological type, grading, and metastasis.Syn and CgAcombined assume a higher sensitivity.Ki-67 can be considered a precursor for effective prognosis .
