目的:探讨糖肾宁对糖尿病肾病大鼠糖脂代谢的影响及其肾脏保护作用的机制。方法:雄性SD大鼠40只,采用腹腔内注射福氏完全佐剂、链脲佐菌素及高脂饲料喂养的方法建立糖尿病肾病大鼠模型。按24h尿微量白蛋白(24hU-Alb)测定值随机分为模型组(M组)、糖肾宁组(T组),格列喹酮组(G组)和正常组(N组)各8只,8周后,检测各组大鼠空腹血糖、空腹胰岛素(INS)、糖化血红蛋白(HbA1c)、三酰甘油(TG),胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、24hU-Alb及肾功能。结果:M组空腹血糖、TC、LDL显著升高,INS、HDL明显下降。治疗8周后T组INS明显上升,TC、LDL显著下降、HDL明显升高,24hU-Alb减少,肾功能显著改善,且明显优于G组。结论:格列喹酮和糖肾宁均可不同程度影响血糖水平,糖肾宁还可有效调节脂代谢。糖肾宁可能是通过调节肾组织糖代谢及血脂代谢,延缓了肾小球硬化及肾间质纤维化进程,发挥了独立于降糖作用之外的降蛋白及肾功能保护作用。

목적:탐토당신저대당뇨병신병대서당지대사적영향급기신장보호작용적궤제。방법:웅성SD대서40지,채용복강내주사복씨완전좌제、련뇨좌균소급고지사료위양적방법건립당뇨병신병대서모형。안24h뇨미량백단백(24hU-Alb)측정치수궤분위모형조(M조)、당신저조(T조),격렬규동조(G조)화정상조(N조)각8지,8주후,검측각조대서공복혈당、공복이도소(INS)、당화혈홍단백(HbA1c)、삼선감유(TG),담고순(TC)、고밀도지단백(HDL)、저밀도지단백(LDL)、24hU-Alb급신공능。결과:M조공복혈당、TC、LDL현저승고,INS、HDL명현하강。치료8주후T조INS명현상승,TC、LDL현저하강、HDL명현승고,24hU-Alb감소,신공능현저개선,차명현우우G조。결론:격렬규동화당신저균가불동정도영향혈당수평,당신저환가유효조절지대사。당신저가능시통과조절신조직당대사급혈지대사,연완료신소구경화급신간질섬유화진정,발휘료독립우강당작용지외적강단백급신공능보호작용。
Objective:To study the effect and mechanism of Tangshenning Recipe on kidney protection from glucose and lipid metabolism in rats with diabetic nepropathy. Methods:50 male SD rats were randomly divided into high fat diet for 3 weeks,was injected intraperitoneally with STZ and CFA at the same time. After 2 weeks,24h U-Alb was measured. According to 24h U-Alb,the 24 SD rats were randomly divided into diabetic model group(group M,n = 8),Tangshenning group(group T,n = 8)and gliquidone group(group G,n = 8). After 8 weeks,the level of fasting blood - glucose,fasting insulin(INS),glycated hemoglobins (HbA1c),triglyceride(TG),cholesterol(TC),high - density lipoprotein(HDL),low density lipoprotein(LDL),creatinine(Cr), urea nitrogen(BUN)and 24h U-Alb was measured. Results:The levels of blood glucose,TC in rats of group M were obviously higher than those of group N. On the other side,the levels of INS and HDL were obviously lower than those of group N. After 8 weeks treatment the levels of INS and HDL were ascended,but TC declined markedly. Corresponding to these,the excretion of 24h U-Alb,Cr and BUN significant improvement of group T. Conclusion:Gliquidone and Tangshenning can regulate blood glucose in differ-ent degree,and the latter can regulate lipid metabolism also. Tangshenning give play to reducing the proteinuria and protecting renal function independent of glucose decrease through the regulation of nephridial tissue glucose metabolism and lipid metabolism in serum.
