目的：研究神经肌肉电刺激联合针刺和吞咽训练治疗脑卒中后吞咽障碍的临床疗效。方法选取2011年7月至2013年7月期我院诊治的70例脑卒中后吞咽障碍患者，根据随机数字法将其分为对照组和观察组各35例。对照组给予基础治疗及肢体康复训练，观察组在对照组治疗的基础上给予神经肌肉电刺激联合针刺和吞咽训练治疗，根据洼田饮水试验、吞咽X线电视透视检查(VFSS)，观察和比较两组临床疗效、治疗前后VFSS评分及不良反应。结果与对照组比较，观察组总有效率明显升高(97.1%vs 77.1%)，治疗后观察组VFSS评分显著升高[(8.0±0.8)分vs (5.2±0.7)分]，不良反应发生率显著降低(11.4%vs 34.3%)，以上各项指标比较差异均有统计学意义(P<0.05)。结论神经肌肉电刺激联合针刺和吞咽训练治疗脑卒中后吞咽障碍患者可明显改善患者的生活质量，值得临床推广。
목적：연구신경기육전자격연합침자화탄인훈련치료뇌졸중후탄인장애적림상료효。방법선취2011년7월지2013년7월기아원진치적70례뇌졸중후탄인장애환자，근거수궤수자법장기분위대조조화관찰조각35례。대조조급여기출치료급지체강복훈련，관찰조재대조조치료적기출상급여신경기육전자격연합침자화탄인훈련치료，근거와전음수시험、탄인X선전시투시검사(VFSS)，관찰화비교량조림상료효、치료전후VFSS평분급불량반응。결과여대조조비교，관찰조총유효솔명현승고(97.1%vs 77.1%)，치료후관찰조VFSS평분현저승고[(8.0±0.8)분vs (5.2±0.7)분]，불량반응발생솔현저강저(11.4%vs 34.3%)，이상각항지표비교차이균유통계학의의(P<0.05)。결론신경기육전자격연합침자화탄인훈련치료뇌졸중후탄인장애환자가명현개선환자적생활질량，치득림상추엄。
Objective To study the clinical efficacy of neuromuscular electrical stimulation combined with acupuncture and swallowing training on dysphagia after stroke. Methods Seventy patients with dysphagia after stroke in our hospital from July 2011 to July 2013 were equally randomized into control group, which were given ba-sic treatment and rehabilitation training, and observation group, which were given additional neuromuscular electrical stimulation combined with acupuncture and swallowing training except basic treatment and rehabilitation training. The clinical efficacy, VFSS scores on pre-and post-treatment and adverse reactions in the two groups according to the Watian drinking water test and swallowing X-ray television fluoroscopy (VFSS) were compared. Results Compared with the control group, the total effective rate significantly increased (97.1% vs 77.1%). And the VFSS scores after treatment obviously increased [(8.0±0.8) vs (5.2±0.7) ]while the incidence of adverse reactions significantly decreased in the observation group (11.4% vs 34.3%), P<0.05. Conclusion For the patients with dysphagia after stroke, the treatment of neuromuscular electrical stimulation combined with acupuncture and swallowing training can significant-ly improve the prognosis quality and is worthy of clinical promotion.
