目的：比较经小隐静脉途径置管直接溶栓及全身溶栓治疗急性下肢深静脉血栓形成（DVT）的临床疗效。方法将65例急性DVT患者根据其意愿分成两组：A组（23例）采用全身抗凝溶栓治疗；B组（42例）经小隐静脉置溶栓导管溶栓治疗。对比分析两组患者治疗后静脉造影结果、患肢膝上15cm及膝下10cm周径变化值、血栓后综合征（PTS）发生率，评价临床疗效。结果 B组治疗后静脉造影的疗效评价、膝上15cm及膝下10cm的周径变化值、PTS发生率均较A组为佳，差异均具有统计学意义（均P＜0.05）。结论经小隐静脉置管直接溶栓治疗急性DVT较全身溶栓能显著减轻肢体肿胀，并可减少PTS的发生，是一种安全、有效且简单的治疗方法。

Objective To compare the efficacy of catheter- directed thrombolysis(CDT) via the smal saphenous veins with systematic thrombolysis for patients with acute deep venous thrombosis (DVT). Methods Sixty five patients with acute DVT ad-mitted from June 2010 to December 2012 were divided into two groups:23 patients received systemic thrombolysis(group A) and 42 patients received CDT via the smal saphenous veins(group B). The imaging findings of limb vein, the circumference of the af-fected legs and the rate of post- thrombotic syndrome (PTS) were examined and compared between two groups before and after treatment. Results The curative efficacy in terms of vein imaging, circumference difference of the affected leg and the rate of PTS in group B were better than those in group A (P<0.05). Conclusion Catheter- directed thrombolysis via the smal saphe-nous veins is an effective, safe and feasible approach to treat acute deep venous thrombosis.
