Direct stripping treatment%Water separation treatment%Overian endometrioma cystec-tomy
目的：探讨直接囊肿剥离术与注水分离囊肿剥离术治疗卵巢子宫内膜异位囊肿临床效果。方法：选取卵巢子宫内膜异位囊肿150例，采用随机数字表法分为对照组（75例）和观察组（75例），分别采用直接法和注水分离法剥离囊肿囊壁；比较两组患者手术时间、术中出血量、囊肿囊壁正常卵巢组织阳性率、卵泡数、手术前后血清卵泡刺激素（follicle stimulating hormone，FSH）、黄体生成素（luteinizing hormone， LH）、雌二醇（estradiol，E2）及抗苗勒管激素（anti M llerian hormone，AMH）水平等。结果：对照组和观察组患者手术时间、术中出血量比较差异无统计学意义（P＞0.05）；观察组患者囊肿囊壁正常卵巢组织阳性率和卵泡数均显著优于对照组，差异有统计学意义（P＜0.05）；两组患者术后FSH、LH、E2和AMH水平均显著优于术前，且观察组患者术后各指标改善程度优于对照组，差异有统计学意义（P＜0.05）。结论：与直接囊肿剥离术相比，注水分离囊肿剥离术治疗卵巢子宫内膜异位囊肿可减少手术对于正常卵巢组织损伤，保存卵巢储备功能，对于提高术后生活质量具有重要意义。

목적：탐토직접낭종박리술여주수분리낭종박리술치료란소자궁내막이위낭종림상효과。방법：선취란소자궁내막이위낭종150례，채용수궤수자표법분위대조조（75례）화관찰조（75례），분별채용직접법화주수분리법박리낭종낭벽；비교량조환자수술시간、술중출혈량、낭종낭벽정상란소조직양성솔、란포수、수술전후혈청란포자격소（follicle stimulating hormone，FSH）、황체생성소（luteinizing hormone， LH）、자이순（estradiol，E2）급항묘륵관격소（anti M llerian hormone，AMH）수평등。결과：대조조화관찰조환자수술시간、술중출혈량비교차이무통계학의의（P＞0.05）；관찰조환자낭종낭벽정상란소조직양성솔화란포수균현저우우대조조，차이유통계학의의（P＜0.05）；량조환자술후FSH、LH、E2화AMH수평균현저우우술전，차관찰조환자술후각지표개선정도우우대조조，차이유통계학의의（P＜0.05）。결론：여직접낭종박리술상비，주수분리낭종박리술치료란소자궁내막이위낭종가감소수술대우정상란소조직손상，보존란소저비공능，대우제고술후생활질량구유중요의의。
Objectives:To investigate the clinical effects of direct stripping treatment and water separation treatment on overian endometrioma cystectomy.Methods:1 50 patients with overian endometrioma cystectomy were chosen and randomly divided into two groups,control group (75 patients)to receive direct stripping treatment and observation group (75 patients)for water separation treatment.The operation time,bleeding volume in operation, positive rate of normal ovarian tissue and follicles number in cyst wall,and levels of FSH,LH,E2 and AMH before and after treatment of both groups were compared.Results:There was no significant difference in operation time and bleeding volume in operation between the two groups (P>0.05);the positive rate of normal ovarian tissue and folli-cles number in cyst wall of the observation group was significant better than control group,with statistically signifi-cant difference (P<0.05).The levels of FSH,LH,E2 and AMH of both groups after treatment were significantly better than these before treatment (P<0.05).The improvement in levels of FSH,LH,E2 and AMH of observation group after treatment was significant better than control group,with statistically significant difference (P<0.05 ). Conclusion:Compared with direct stripping treatment,water separation treatment on overian endometrioma cystec-tomy can efficiently relieve the injury of normal ovarian tissue,protect ovarian reservation and is essential to improve the life quality after surgery.
