目的:探讨早期使用胰岛素治疗糖尿病微量白蛋白尿的效果。方法将80例早期糖尿病肾病患者随机均分为观察组和对照组,对照组给予格列喹酮口服治疗,观察组给予甘精胰岛素联合格列喹酮口服治疗,观察两组患者的治疗效果。结果观察组治疗后的FBG、2 h PG及HbA1c水平均显著低于对照组(P<0.05),治疗后的24 h尿微量白蛋白、Homa-IR及ACR显著低于对照组(P<0.05);两组患者治疗期间的低血糖发生率比较,差异无统计学意义(P>0.05)。结论早期应用胰岛素治疗糖尿病微量白蛋白尿,可在降低血糖的同时减少尿白蛋白排泄量,对延缓糖尿病肾病的发展有重要意义。

Objective To explore the curative effects of early application of insulin in the treatment of diabetic mi-croalbuminuria. Methods 80 patients with early diabetic nephropathy were randomly and evenly allocated to the obser-vation group and the control group.The control group was orally given gliquidone,and the observation group was orally given insulin glargine combined with gliquidone.Curative effects were observed in the two groups. Results FBG,2 h PG and HbA1c level in the observation group after the treatment were significantly lower than those in the control group (P<0.05); 24 h urine micro-albumin,Homa-IR and ACR level in the observation group after the treatment were signifi-cantly lower than those in the control group (P<0.05);the difference of the incidence of hypoglycemia between the two groups was not statistically significant (P>0.05). Conclusion Early application of insulin in the treatment of diabetic microalbuminuria helps lower both the blood sugar and the excretion of urinary albumin,which has a great significance in delaying the development of diabetic nephropathy.
