目的：探讨监测血浆降钙素原（PCT）的水平对诊断肺结核合并感染病的临床效果。方法：对2012年1月~2013年12月期间我院收治的1998例肺结核患者的临床资料进行回顾性研究。我们对这1998例患者进行了两周的血浆降钙素原水平的监测，对其中血浆降钙素原水平较高或出现疑似合并感染病的患者进行细菌培养，观察患者感染病菌的情况。再根据病菌感染情况对比肺结核患者的血浆降钙素原的水平。结果：经过监测，在1998例肺结核患者中，有136例患者合并其他感染，其感染率为6.8%。在这136例肺结核伴有感染的患者中，有78例患者感染了革兰氏阴性菌，有30例患者感染了革兰氏阳性菌，有28例患者感染了真菌。结论：对肺结核患者进行血清降钙素原水平的监测，有助于诊断肺结核合并其他感染。此方法值得在临床上推广使用。
목적：탐토감측혈장강개소원（PCT）적수평대진단폐결핵합병감염병적림상효과。방법：대2012년1월~2013년12월기간아원수치적1998례폐결핵환자적림상자료진행회고성연구。아문대저1998례환자진행료량주적혈장강개소원수평적감측，대기중혈장강개소원수평교고혹출현의사합병감염병적환자진행세균배양，관찰환자감염병균적정황。재근거병균감염정황대비폐결핵환자적혈장강개소원적수평。결과：경과감측，재1998례폐결핵환자중，유136례환자합병기타감염，기감염솔위6.8%。재저136례폐결핵반유감염적환자중，유78례환자감염료혁란씨음성균，유30례환자감염료혁란씨양성균，유28례환자감염료진균。결론：대폐결핵환자진행혈청강개소원수평적감측，유조우진단폐결핵합병기타감염。차방법치득재림상상추엄사용。
Objective To study the levels of serum procalcitonin for monitoring of nosocomial infection in patients with pulmonary tuberculosis, in order to control infection. Methods The procalcitonin was detected every two weeks in 1998 cases of hospitalized patients with pulmonary tuberculosis, and combined with clinical symptoms and lower respiratory tract specimens results in isolation and culture, analysis of hospital infection. Results According to isolated pathogens, hospital infection rate was 6.91%in patients with pulmonary tuberculosis. Detection of PCT levels of pulmonary tuberculosis patients with no hospital infection was lower than of pulmonary tuberculosis patients with hospital infection;PCT levels of pulmonary tuberculosis patients with gram negative bacilli was significantly higher than that of positive cocci . t’=8.9654,t’α=2.0086,P<0.05.Conclusion Dynamic observation of serum procalcitonin level can timely find patients in hospital infection, and according to the elevation of PCT level preliminary judgment of infection type, In order to facilitate timely control.
