目的:评价 OSAHS 对肺动脉压及右心功能的影响。方法选择 OSAHS 患者72例和正常鼾症组23例,分别行多导睡眠呼吸监测和超声心动图测定心脏结构、血流动力学参数并估算肺动脉压。结果72例 OSAHS 患者中肺动脉高压(PH)的发生率为38.89%,23例鼾症组中 PH 的发病率为17.39%,且 OSHAS 组患者右心结构及血流动力学变化程度均明显高于鼾症组(P <0.05或 P <0.01),OSAHS 患者肺动脉压、右心结构及血流动力学变化与 OSAHS 严重程度相关。结论 OSAHS患者中 PH 的发生率较高,OSAHS 同时合并 PH 的患者其右心结构变化较明显;OSAHS 患者的肺动脉压力升高程度和右心结构变化程度与 OSAHS 的严重程度呈正相关。

목적:평개 OSAHS 대폐동맥압급우심공능적영향。방법선택 OSAHS 환자72례화정상한증조23례,분별행다도수면호흡감측화초성심동도측정심장결구、혈류동역학삼수병고산폐동맥압。결과72례 OSAHS 환자중폐동맥고압(PH)적발생솔위38.89%,23례한증조중 PH 적발병솔위17.39%,차 OSHAS 조환자우심결구급혈류동역학변화정도균명현고우한증조(P <0.05혹 P <0.01),OSAHS 환자폐동맥압、우심결구급혈류동역학변화여 OSAHS 엄중정도상관。결론 OSAHS환자중 PH 적발생솔교고,OSAHS 동시합병 PH 적환자기우심결구변화교명현;OSAHS 환자적폐동맥압력승고정도화우심결구변화정도여 OSAHS 적엄중정도정정상관。
Objective To evaluate obstructive sleep apnea hypopnea syndrom (OSAHS) on pulmonary hypertension (PH)and right heart function.Methods Seventy-two patients with OSAHS and 23 snoring patients were determined by polysomnography and echocardiographic to evaluate heart structure,hemodynamic parameters measurement and pulmonary hypertension PH.Results Seventy-two patients with OSAHS in the incidence of PH,right heart structure and hemodynamic changes were significantly higher than that of 23 snoring patients (P <0.05 or P <0.01),OSAHS patients with PH, right heart structure and hemodynamic changed good correlation with OSAHS severity.Conclusions The incidence of OSAHS patients with PH was higher,patients with OSAHS combined PH at the same time whose right heart structure changed was obvious.Severity of PH and right heart structure was changed with the severity of OSAHS.
