目的:探讨脂联素与 COPD 患者血清脂联素水平与 IL-2、IL-8的相关性,以明确脂联素在COPD 慢性炎症中的作用。方法选取正常体质量指数(<24 kg/m2)的男性健康志愿者30名作为对照组,COPD 急性加重期30例患者,稳定期30例患者。COPD 的诊断标准符合中华人民共和国卫生部颁发的《慢性阻塞性肺疾病诊断标准(2013年修订版)》。采用酶联免疫吸附试验(Enzyme-linked Immunosorbent Assay,ELISA)测定3组血清脂联素浓度及 IL-2、IL-8的含量,比较 COPD 急性加重期、稳定期与对照组脂联素、IL-2、IL-8的差异,分析血清脂联素与 IL-2、IL-8的相关性。结果 COPD 急性加重期、稳定期血清脂联素、IL-8显著高于对照组,差异有统计学意义(P <0.01),稳定期血清脂联素浓度及 IL-8显著高于对照组,差异有统计学意义(P <0.01);COPD 急性加重期及稳定期 IL-2显著低于对照组(P <0.01),且 COPD 急性加重期血清 IL-2浓度较稳定期进一步降低,差异有统计学意义(P <0.01)。COPD 急性加重期及稳定期患者血清脂联素与 IL-2呈负相关,差异有统计学意义(相关系数 r分别为-0.654,-0.643,P <0.05),与 IL-8呈正相关,差异具有统计学意义(相关系数 r 分别为0.545,0.649,P <0.01)。结论血清脂联素在 COPD 发病机制过程中起重要作用,推测血清脂联素可能是COPD 新型的促炎因子。

목적:탐토지련소여 COPD 환자혈청지련소수평여 IL-2、IL-8적상관성,이명학지련소재COPD 만성염증중적작용。방법선취정상체질량지수(<24 kg/m2)적남성건강지원자30명작위대조조,COPD 급성가중기30례환자,은정기30례환자。COPD 적진단표준부합중화인민공화국위생부반발적《만성조새성폐질병진단표준(2013년수정판)》。채용매련면역흡부시험(Enzyme-linked Immunosorbent Assay,ELISA)측정3조혈청지련소농도급 IL-2、IL-8적함량,비교 COPD 급성가중기、은정기여대조조지련소、IL-2、IL-8적차이,분석혈청지련소여 IL-2、IL-8적상관성。결과 COPD 급성가중기、은정기혈청지련소、IL-8현저고우대조조,차이유통계학의의(P <0.01),은정기혈청지련소농도급 IL-8현저고우대조조,차이유통계학의의(P <0.01);COPD 급성가중기급은정기 IL-2현저저우대조조(P <0.01),차 COPD 급성가중기혈청 IL-2농도교은정기진일보강저,차이유통계학의의(P <0.01)。COPD 급성가중기급은정기환자혈청지련소여 IL-2정부상관,차이유통계학의의(상관계수 r분별위-0.654,-0.643,P <0.05),여 IL-8정정상관,차이구유통계학의의(상관계수 r 분별위0.545,0.649,P <0.01)。결론혈청지련소재 COPD 발병궤제과정중기중요작용,추측혈청지련소가능시COPD 신형적촉염인자。
Objective To explore relationship between adiponectin and IL-2,IL-8 in chronic obstructive pulmonary disease (COPD)in order to make sure the role in chronic inflammation of COPD. Methods To select 30 healthy people of normal body mass index (BMI < 24 kg/m2 )as controls,30 patients as an acute exacerbation group and 30 patients as a stable group.All subjects with COPD were diagnosed accrodding to COPD diagnosis and treatment guidelines (revised in 2013),and Enzyme-linked Immunosorbent Assay (ELISA)was used to determine levels of serum adiponectin,IL-2 and IL-8,and then difference needs to be compared among these groups to analyze relationship between adiponectin and IL-2,IL-8.Results Levels of serum adiponectin and IL-8 in acute exacerbation group and stable were significantly higher than the control group (P < 0.01 ).Further,levels of serum adiponectin and IL-8 in acute exacerbation group was also significantly higher than concentrations in stable group with COPD (P <0.01).However,levels of IL-2 in controls was higher than acute exacerbation and stable groups′(P <0.01).And levels of IL-2 in actue exacerbation group was less than concentrations of IL-2 in stable group with COPD (P <0.01).Concentrations of serum adiponectin in stable and actue exacerbation group with COPD was negative correlation with IL-2 (coefficient of association r respectively were -0.654 and -0.643,P <0.01)and positive correlation with IL-8 (coefficient of association r respectively were 0.545 and 0.649,P <0.01).Conclusions Serum adiponectin might play an important role in the chronic inflammation of COPD as a new proinflammatory factor.
