目的：探究高血压伴发合并症患者的中心动脉压与血管功能参数的变化。方法入选对象为2009年10月至2013年3月期间在瑞金医院老年病科接受无创中心动脉压检测的志愿者344例，其中正常对照组124例，单纯高血压达标组67例，高血压伴发合并症组153例。所有入组对象采用BPro?动脉脉搏波采集设备，结合A-PULSE CASP?中心动脉压应用软件，采用改良型扁平张力法专利技术，实时监测无创中心动脉压，并检测动脉反射波增强指数（RAI）、动脉反射波与收缩期巅峰之时差（PRT）等血管功能参数。结果高血压伴发合并症降压达标组CASP高于正常对照组（P＜0.01），与单纯高血压降压达标组差异无统计学意义，血管功能指标均无显著差异；高血压伴发合并症患者中，降压达标与降压未达标两组比较，前者CSAP明显低于后者，两者差异有统计学意义（P＜0.01）。结论高血压伴发合并症患者积极控制外周血压有助于降低中心动脉压及改善血管功能，且中心动脉压与血管功能监测可以用于预测降压达标与否的临床预后。
목적：탐구고혈압반발합병증환자적중심동맥압여혈관공능삼수적변화。방법입선대상위2009년10월지2013년3월기간재서금의원노년병과접수무창중심동맥압검측적지원자344례，기중정상대조조124례，단순고혈압체표조67례，고혈압반발합병증조153례。소유입조대상채용BPro?동맥맥박파채집설비，결합A-PULSE CASP?중심동맥압응용연건，채용개량형편평장역법전리기술，실시감측무창중심동맥압，병검측동맥반사파증강지수（RAI）、동맥반사파여수축기전봉지시차（PRT）등혈관공능삼수。결과고혈압반발합병증강압체표조CASP고우정상대조조（P＜0.01），여단순고혈압강압체표조차이무통계학의의，혈관공능지표균무현저차이；고혈압반발합병증환자중，강압체표여강압미체표량조비교，전자CSAP명현저우후자，량자차이유통계학의의（P＜0.01）。결론고혈압반발합병증환자적겁공제외주혈압유조우강저중심동맥압급개선혈관공능，차중심동맥압여혈관공능감측가이용우예측강압체표여부적림상예후。
ObjectiveTo investigate the alteration of central aortic systolic pressure (CASP) and vascular function in the patients with complicated hypertension.Methods A total of 344 volunteers undergoingnoninvasive arterial pressure detection intheDepartmentof Geriatrics of Ruijin Hospital from October 2009 to March 2013 were recruited in this study. They were divided into 3 groups: the normal control group (n=124), controlled hypertension group(n=67) and complicated hypertension group (n=153). CASP,radialartery augmentation index of reflected wave(RAI), andpeak relative time(PRT) were measured non-invasively by radial artery applanation tonometry BPro? (A-PULSE CASP? and corresponding software).ResultsCASPin the complicated hypertensionpatientswhose blood pressurewell-controlledwas higher thanthat inthe normal controlgroup (P<0.01), andhad nosignificant difference with that of controlled hypertension group.There was no difference in vascular function betweenthe 2groups.Forthe complicated hypertensionpatients, CASP was significantly lower in those with well-controlled blood pressurethaninthose without (P<0.01).Conclusion Well controlling blood pressure in thepatients withcomplicatedhypertension is helpfulto reducethe CASP and improve the vascular function.And, monitoring CASP and vascular function can be used topredict clinical prognosis ofblood pressurecontrol.
