目的:探讨超声在诊断类风湿性关节炎(RA)患者膝关节软组织病变及关节积液的应用价值。方法 RA组患者57例,体检健康者57例为对照组,超声观测两组膝关节软组织病变及关节积液的情况。结果 RA组髌上囊液体深度(0.58±0.29)cm,滑膜厚度(0.41±0.32)cm,均大于对照组;股骨内、外侧髁软骨厚度为(0.76±0.25)mm和(1.12±0.23)mm,均小于对照组,差异均有统计学意义(均P<0.05)。RA组中髌上囊积液检出率为84.2%,高于对照组(8.7%);RA组滑膜内血流检出率为80.7%,亦高于对照组(3.5%),差异均有统计学意义(均P<0.05)。结论超声对RA患者膝关节软组织病变及关节积液的诊断准确,具有无创、直观及重复性好等优点,值得推广。

목적:탐토초성재진단류풍습성관절염(RA)환자슬관절연조직병변급관절적액적응용개치。방법 RA조환자57례,체검건강자57례위대조조,초성관측량조슬관절연조직병변급관절적액적정황。결과 RA조빈상낭액체심도(0.58±0.29)cm,활막후도(0.41±0.32)cm,균대우대조조;고골내、외측과연골후도위(0.76±0.25)mm화(1.12±0.23)mm,균소우대조조,차이균유통계학의의(균P<0.05)。RA조중빈상낭적액검출솔위84.2%,고우대조조(8.7%);RA조활막내혈류검출솔위80.7%,역고우대조조(3.5%),차이균유통계학의의(균P<0.05)。결론초성대RA환자슬관절연조직병변급관절적액적진단준학,구유무창、직관급중복성호등우점,치득추엄。
Objective To investigate the value of high-frequency ultrasound in the diagnosis of rheumatoid arthritis patients with knee joint effusion and soft tissue lesions. Methods A total of 57 patients with rheumatoid arthritis were chosen as the observation group,57 healthy volunteers as control group. All the subjects underwent high-frequency ultrasound examination and the knee joint soft tissue lesions and joint effusion situation in the two groups were observed and compared. Results The patellar bursa fluid depth and synovial thickness in observation group were(0.58±0.29)cm and(0.41±0.32)cm,which were all higher than those in the control group. However,the femur and lateral condyle cartilage thickness were(0.76±0.25)cm and(1.12±0.23)mm, which were all less than the control group(P<0.05). The detection rate of patellar bursa fluid and synovial blood flow were 84.2%and 80.7%,respectively,which were significantly higher than those in the control group(8.7%,3.5%). Conclusion The high frequency ultrasound can accurately diagnosis the rheumatoid arthritis with advantages of non-invasiveness,intuition and good repeatability,and be worthy of clinical further promotion.
