目的:观察2型糖尿病患者不同病程阶段经颅多普勒超声( transcranial Doppler sonograghy,TCD)变化,探讨糖尿病病程与脑血管病变的关系。方法选择2型糖尿病患者80例,根据病程分为2组,A 组(病程≥10年)40例,B 组(病程﹤10年)40例,并选择健康体检者40例作为对照组(C 组)。行 TCD 检查,比较3组左右两侧大脑中动脉(middle cerebral artery,MCA)及左右两侧椎动脉(vertebral artery,VA)的收缩期峰值血流速度及搏动指数(pulseindex,PI)。结果 A 组和 B 组左右两侧 MCA 及左右两侧 VA 的收缩期峰值血流速度明显高于 C 组(P ﹤0.01),A 组左右两侧 MCA 及左右两侧 VA 的收缩期峰值血流速度明显高于 B 组(P ﹤0.01);A 组左右两侧 MCA及左右两侧 VA 的 PI 值明显高于 B 组和 C 组(P ﹤0.01),B 组左右两侧 MCA 及左右两侧 VA 的 PI 值与 C 组比较差异无统计学意义(P ﹥0.05)。结论糖尿病患者脑血管病变与糖尿病病程具有一定的关系,TCD 检查提供了脑血管功能状态的重要信息。

목적:관찰2형당뇨병환자불동병정계단경로다보륵초성( transcranial Doppler sonograghy,TCD)변화,탐토당뇨병병정여뇌혈관병변적관계。방법선택2형당뇨병환자80례,근거병정분위2조,A 조(병정≥10년)40례,B 조(병정﹤10년)40례,병선택건강체검자40례작위대조조(C 조)。행 TCD 검사,비교3조좌우량측대뇌중동맥(middle cerebral artery,MCA)급좌우량측추동맥(vertebral artery,VA)적수축기봉치혈류속도급박동지수(pulseindex,PI)。결과 A 조화 B 조좌우량측 MCA 급좌우량측 VA 적수축기봉치혈류속도명현고우 C 조(P ﹤0.01),A 조좌우량측 MCA 급좌우량측 VA 적수축기봉치혈류속도명현고우 B 조(P ﹤0.01);A 조좌우량측 MCA급좌우량측 VA 적 PI 치명현고우 B 조화 C 조(P ﹤0.01),B 조좌우량측 MCA 급좌우량측 VA 적 PI 치여 C 조비교차이무통계학의의(P ﹥0.05)。결론당뇨병환자뇌혈관병변여당뇨병병정구유일정적관계,TCD 검사제공료뇌혈관공능상태적중요신식。
Objective To explore the relationship between duration of diabetes mellitus and cerebrovascular disease by observing the variation of transcranial Doppler sonograghy(TCD)in different stages of type 2 diabetes mellitus patients. Methods Eighty type 2 diabetes mellitus patients who were hospitalized or confirmed in the department of our hospital were divided to two groups,40 patients in group A(disease course≥10 years)and 40 patients in group B(disease course ﹤ 10 years). The group C including 40 health persons with similar age served as control group( group C). The systolic peak velocity(VP)and pulsatility index(PI)of middle cerebral artery(MCA)of left and right sides and vertebral artery(VA)of left and right sides in three groups were detected by TCD. Results The VP of MCA of left and right sides and VA of left and right sides of group A and group B were significantly higher than those of group C(P ﹤ 0. 01),the VP of MCA of left and right sides and VA of left and right sides of group A were much higher than those of group B(P ﹤ 0. 01),the PI of MCA of left and right sides and VA of left and right sides of group A were much higher than those of group B and group C(P ﹤ 0. 01), and there were no significant statistic differences between the PI of MCA of left and right side and VA of left and right side of group B and group C(P ﹥ 0. 05). Conclusion The cerebrovascular disease may be related to disease course of diabetes mellitus patients and TCD test can provide important information in functional status of cerebral blood vessels.
