目的:探讨膨肺吸痰法联合胸肺物理治疗在重型颅脑损伤机械通气患者中的应用效果。方法:将60例严重颅脑损伤行机械通气治疗的患者按完全随机数字表法分为观察组与对照组,两组均给予机械通气、脱水、降颅压、抗感染、化痰及对症支持治疗。除上述治疗外,观察组给予体位引流、胸肺物理治疗及膨肺吸痰,对照组给予翻身叩背和常规吸痰,比较两组患者治疗5 d后肺功能指标、累计总吸痰次数和肺不张、肺部感染的发生情况。结果:通过膨肺吸痰联合胸肺物理治疗,观察组吸痰后肺功能明显改善,吸痰效果及并发症发生率明显优于对照组,比较差异有统计学意义(P<0.05)。结论:膨肺吸痰联合胸肺物理治疗能改善重型颅脑损伤患者的低氧状况,促进肺复张,减少肺部感染,是一种安全有效的气道分泌物清除方式。

목적:탐토팽폐흡담법연합흉폐물리치료재중형로뇌손상궤계통기환자중적응용효과。방법:장60례엄중로뇌손상행궤계통기치료적환자안완전수궤수자표법분위관찰조여대조조,량조균급여궤계통기、탈수、강로압、항감염、화담급대증지지치료。제상술치료외,관찰조급여체위인류、흉폐물리치료급팽폐흡담,대조조급여번신고배화상규흡담,비교량조환자치료5 d후폐공능지표、루계총흡담차수화폐불장、폐부감염적발생정황。결과:통과팽폐흡담연합흉폐물리치료,관찰조흡담후폐공능명현개선,흡담효과급병발증발생솔명현우우대조조,비교차이유통계학의의(P<0.05)。결론:팽폐흡담연합흉폐물리치료능개선중형로뇌손상환자적저양상황,촉진폐복장,감소폐부감염,시일충안전유효적기도분비물청제방식。
Objective:To investigate the effect of lung dilated suction with physical therapy in severe craniocerebral injury patients during mechanical ventilation.Method: 60 severe craniocerebral injury patients were divided into two groups randomly, both groups were treated with mechanical ventilation, dehydration, intracranial pressure reduction, anti-infection and supportive treatment. In addition to all of above, decreasing intracranial pressure of the observation group was treated with postural drainage, lung physical therapy, and lung dilated suction, while the control group was treated with back-percussion andpercussion on back percussion on backroutine suctioning. After five-days’ treatment, the indexes of lung function, the total number of suction, rate of atelectasis and lung infection were compared. Result: Pass through the Lung dilated suction with physical therapy, there was significant improvement in indexes of lung function in the observation group (P<0.05), the suction effects and complication rate were superior to the control group (P<0.05).Conclusion: Lung dilated suction with lung physical therapy can improve oxygenated function, promote lung recruitment, and reduce lung infection. It’s safe and effective way to clean airway secretion.
