目的：临床分析PCNL在鹿角形肾结石治疗中的应用效果。方法随机选取我院2012年1月至2013年6月间实施治疗的68例鹿角形肾结石患者，并将其平均分成两组，对照组患者实施开放手术治疗，观察组患者实施PCNL治疗，对比两组患者的临床治疗效果。结果观察组患者的手术时间、术中出血量、住院时间均明显少于对照组，差异显著（P＜0．05）；观察组患者的结石取净率、复发率、肾功能不全发生率以及切口周围不适发生率均明显低于对照组，差异显著（P＜0．05）；观察组患者的经济花费明显高于对照组，差异显著（P＜0．05）。结论 PCNL治疗鹿角形肾结石效果显著，值得推广，但其手术费用较高。
목적：림상분석PCNL재록각형신결석치료중적응용효과。방법수궤선취아원2012년1월지2013년6월간실시치료적68례록각형신결석환자，병장기평균분성량조，대조조환자실시개방수술치료，관찰조환자실시PCNL치료，대비량조환자적림상치료효과。결과관찰조환자적수술시간、술중출혈량、주원시간균명현소우대조조，차이현저（P＜0．05）；관찰조환자적결석취정솔、복발솔、신공능불전발생솔이급절구주위불괄발생솔균명현저우대조조，차이현저（P＜0．05）；관찰조환자적경제화비명현고우대조조，차이현저（P＜0．05）。결론 PCNL치료록각형신결석효과현저，치득추엄，단기수술비용교고。
Objectives To investigate the clinical effects on the results of percutaneous nephrolithotomy (PCNL)for staghorn calculi. Methods We retrospectively analyzed the hospital records who underwent PCNL or open surgery for staghorn calculi at our hospital from January 2012 to June 2013 . 68 patients were randomly se-lected and divided into two groups according to the difference of treatment method:observation group for patients with PCNL treatment (n=34)and control group (n=34)for patients with open surgery. Then we compared two groups of patients with clinical efficacy and complications. Results Observation group had significantly less operative time,blood loss and hospital stay time than control group (P<0. 05),it also has significantly higher stone free rate,lower relapse rate,incidence of renal insufficiency and the occurring rate of discomfort around the incision than control group (P<0 . 05 ). Economic costs of observation group were significantly higher than control group (P<0 . 05 ). Conclusions PCNL for treatment of staghorn calculi has remarkable effect and is wor-thy of promotion,however it is costly.
