目的：比较研究单节段L4/L5椎间盘突出引起单纯单侧感觉或运动异常时突出髓核在MRI上的分布差异。方法回顾分析22例单节段L4/L5椎间盘突出并单侧单纯感觉或运动异常的患者的MRI影像资料，分为运动异常组（A组）和感觉异常组（B组），分别将2组突出髓核的的外侧缘、内侧缘、突出最顶点在横向位上赋值，将突出最顶点在矢状位上赋值，比较2组间赋值的差异。结果 B组突出髓核的外侧缘及突出最顶点，突出髓核的最内侧缘较A组更偏向有症状侧，2组之间有明显差异，B组突出最顶点的矢状位赋值较A组的大。结论感觉异常患者较运动异常患者的突出髓核更偏向受压迫的神经根，且突出髓核在矢状位上更大。
목적：비교연구단절단L4/L5추간반돌출인기단순단측감각혹운동이상시돌출수핵재MRI상적분포차이。방법회고분석22례단절단L4/L5추간반돌출병단측단순감각혹운동이상적환자적MRI영상자료，분위운동이상조（A조）화감각이상조（B조），분별장2조돌출수핵적적외측연、내측연、돌출최정점재횡향위상부치，장돌출최정점재시상위상부치，비교2조간부치적차이。결과 B조돌출수핵적외측연급돌출최정점，돌출수핵적최내측연교A조경편향유증상측，2조지간유명현차이，B조돌출최정점적시상위부치교A조적대。결론감각이상환자교운동이상환자적돌출수핵경편향수압박적신경근，차돌출수핵재시상위상경대。
Objective To compare the nucleus pulposus distribution on MRI in patients with unilateral sensory or motor abnormal caused by lumbar disc herniation of L 4/L5 .Methods A total of 22 patients of L 4/L5 disc herniation with unilateral sensory or motor abnormal were found , and their MRI were collected .The patients were divided into 2 groups:unilateral motor abnormal group(Group A) and unilateral sensory abnormal group (Group B).The outer lateral edge, the inner lateral edge and the vertex of the nucleus pulposus were assigned in the horizontal position .The vertex of the nucleus pulposus was also assigned in the sagittal position .Then Assignment scores of the 2 groups were compared .Results Compared with Group A , Group B showed that the outer lateral edge , the inner lateral edge and the vertex of the nucleus pulposus were more close to the side with symptoms in the transverse section .The sagittal scores of the vertex of the nucleus pulposus in Group B were lager than that in the Group A .Conclusion The nucleus pulposus of unilateral sensory abnormal patients are close to the com -pressed nerve root , and its distribution is larger in the sagittal position , compared with motor abnormal patients .
