目的：探讨多形性未分化肉瘤的M R表现，提高对该病的诊断水平。方法收集经手术、病理证实的患者11例12个病灶，所有病例均行M R检查。结果11例多形性未分化肉瘤，大腿6例，小腿1例，上臂1例，肩背部1例，臀部1例，腹膜后1例；4个边缘清楚，8个边缘模糊，周围肌肉软组织均可见不同程度的水肿表现。其中10个信号不均匀，8个T1 WI表现为等、低信号，1个病灶内可见少高信号，提示肿瘤内合并出血成分，T2 WI抑脂序列所有肿瘤的实质部分均呈不均匀等或稍高信号，囊变坏死区可表现为明显高信号，出血区呈低信号；2个病灶呈软组织肿块，信号尚均匀，T1 WI呈等信号，T2 WI呈稍高信号；增强扫描后呈实质部分明显强化。结论多形性未分化肉瘤的M R表现具有一定的特征性，MR对多形性未分化肉瘤有一定的诊断价值。
목적：탐토다형성미분화육류적M R표현，제고대해병적진단수평。방법수집경수술、병리증실적환자11례12개병조，소유병례균행M R검사。결과11례다형성미분화육류，대퇴6례，소퇴1례，상비1례，견배부1례，둔부1례，복막후1례；4개변연청초，8개변연모호，주위기육연조직균가견불동정도적수종표현。기중10개신호불균균，8개T1 WI표현위등、저신호，1개병조내가견소고신호，제시종류내합병출혈성분，T2 WI억지서렬소유종류적실질부분균정불균균등혹초고신호，낭변배사구가표현위명현고신호，출혈구정저신호；2개병조정연조직종괴，신호상균균，T1 WI정등신호，T2 WI정초고신호；증강소묘후정실질부분명현강화。결론다형성미분화육류적M R표현구유일정적특정성，MR대다형성미분화육류유일정적진단개치。
Objective To investigated the MR appearance of undifferentiated pleomorphic sarcoma .Methods 12 lesions of eleven cases with pathologically confirmed UPS were analyzed retrospectively .All cases were performed MR examina-tion .Results Of the 11 cases of pleomorphic undifferentiated sarcoma ,6 cases were located in thigh ,1 case in calf ,1 case in upper arm ,1 case in humeral back ,1 case in retroperitoneal .Four lesions presented with clear boundary ,eight with unclear edge ,all the leisions with the different degree of edema in the arround soft tissue .10 lesions were inhomogeneous signal ,eight lesions were shown low ,iso-signals intensity on T1WI ,high signal can be seen in one lesion ,which prompt tumor with bleeding ,isointensity or slightly high signal intensity on T 2 WI fat suppression sequence ,necrotic area can be characterized by high signal obviously ,bleeding area with low signal .The even signals can be seen in 2 cases ,slightly high signal on T2-weighted imaging .The sloid parts on the tumors had been obvious enhancement .Conclusion MR manifesta-tion of pleomorphic undifferentiated sarcoma has certain characteristics .MR has a certain value in the diagnosis of undif-ferentiated pleomorphic sarcoma .
