目的：探讨胃低张碳酸饮料充气法螺旋CT扫描与胃镜在胃癌诊断中的临床价值分析。方法选择经手术病理确诊的老年胃癌患者80例，检查前患者均饮用100～300ml碳酸饮料。结果80例患者中有9例因病变导致胃腔狭窄，胃镜没法通过，因此无法判断病变的范围、形状及不能完成活组织检查；有5例患者因高龄、体弱等原因不能耐受胃镜检查而放弃。其余患者均顺利完成检查并取活组织行病理检查；所有患者均行CT检查。对同时接受胃镜检查与螺旋CT检查的患者进行比较，早期胃癌诊断胃镜优于螺旋CT ，进展期胃癌两者检查率相当，而对于判断术前分期，螺旋CT具有较大的优势。结论螺旋CT较胃镜对于老年患者具有良好的耐受性，并且能够显示整个胃壁及准确分析肿瘤的位置和形态结构，还可观察胃周的淋巴结转移情况；然而胃镜对于早期胃癌的诊断较螺旋CT更准确，两者结合，可提高诊断率。
목적：탐토위저장탄산음료충기법라선CT소묘여위경재위암진단중적림상개치분석。방법선택경수술병리학진적노년위암환자80례，검사전환자균음용100～300ml탄산음료。결과80례환자중유9례인병변도치위강협착，위경몰법통과，인차무법판단병변적범위、형상급불능완성활조직검사；유5례환자인고령、체약등원인불능내수위경검사이방기。기여환자균순리완성검사병취활조직행병리검사；소유환자균행CT검사。대동시접수위경검사여라선CT검사적환자진행비교，조기위암진단위경우우라선CT ，진전기위암량자검사솔상당，이대우판단술전분기，라선CT구유교대적우세。결론라선CT교위경대우노년환자구유량호적내수성，병차능구현시정개위벽급준학분석종류적위치화형태결구，환가관찰위주적림파결전이정황；연이위경대우조기위암적진단교라선CT경준학，량자결합，가제고진단솔。
Objective This paper aims to explore the clinic value in the diagnosis of gastric cancer between the stomach carbonated drinks inflatable France hypotonic spiral CT and endoscopy .Methods We chose 80 cases who were pathologi-cally diagnosed gastric cancer from January 2011 to December 2012 in our hospital .We compared the clinic date of the pa-tients who accepted both the stomach carbonated drinks inflatable France hypotonic spiral CT and endoscopy .Before tak-ing spiral CT ,the patients should drink 100~300 ml carbonated beverages .Results Because of the gastric stenosis ,there were 9 cases that endoscopy could not pass .So it could not determine the lesions’ range ,shape and can not complete the biopsy ;Because of age ,infirmity and other reasons ,there were another 5 patients could not be tolerated gastroscopy and give up .The remaining patients were checked successfully and living tissue for pathological examination ;All patients un-derwent CT examnation .Compared with two examinations ,spiral CT was superior to endoscopy in diagnosis of early gas-tric cancer whereas ,it was approximately the same ratio in advanced gastric cancer .For preoperative staging of judgment , spiral CT had a large advantage .Conclusion Compared with endoscopy ,spiral CT had a good tolerability to elderly pa-tients and could display the entire stomach ,accurate analysis of tumor’ location and observe morphology’s gastric lymph node metastasis .However ,compared with spiral CT ,endoscopy is more accurate in the diagnosis of early gastric cancer . So physicians should choose the right methord based on specific clinical situation .By use of proper inspection methods ra-tionally ,the suffering of patients can be adleviated effectively .
