目的：采用 EuroQol五维调查问卷（EQ-5D）对帕金森病患者的生活质量进行评价，并分析其影响因素。方法选择帕金森病患者52例，采用 EQ-5D、统一帕金森病评定量表（unified Parkinson's diseasc rating scale ， UPDRS）、Hoehn-Yahr（H-Y）分级、帕金森病睡眠量表（Parkinson's disease sleep scale ，PDSS）、汉密尔顿抑郁量表（HAMD）和汉密尔顿焦虑量表（HAMA），对帕金森病患者的生活质量和健康状况进行分析。结果帕金森病各评定量表间相关分析显示，失眠（r＝-0．519）、UPDRS Ⅰ（r＝-0．568）、UPDRS Ⅱ（r＝-0．705）、UPDRS Ⅲ（r＝-0．760）、UPDRS Ⅳ（r＝-0．571）、HAMD（r＝-0．669）、HAMA（r＝-0．549）、H-Y分级（r＝-0．694）、PDSS （r＝1．000）、病程（r＝-0．640）、文化程度（r＝0．293）与帕金森病患者生活质量有关（P＜0．01）。多元回归分析显示，影响EQ-5D 指数因素有 UPDRS Ⅰ、UPDRS Ⅲ、HAMD、HAMA 和 PDSS ，其中 UPDRS Ⅰ、UPDRS Ⅲ、HAMA和 HAMD与EQ-5D指数呈负相关（P＜0．05，P＜0．01）；PDSS与EQ-5D指数呈正相关（P＜0．01）。结论EQ-5D适用于帕金森病患者生活质量研究，临床医师在重视帕金森病患者的诊断和运动症状治疗前提下，还应重视患者睡眠、焦虑抑郁等非运动症状，尽量提高患者生活质量。
목적：채용 EuroQol오유조사문권（EQ-5D）대파금삼병환자적생활질량진행평개，병분석기영향인소。방법선택파금삼병환자52례，채용 EQ-5D、통일파금삼병평정량표（unified Parkinson's diseasc rating scale ， UPDRS）、Hoehn-Yahr（H-Y）분급、파금삼병수면량표（Parkinson's disease sleep scale ，PDSS）、한밀이돈억욱량표（HAMD）화한밀이돈초필량표（HAMA），대파금삼병환자적생활질량화건강상황진행분석。결과파금삼병각평정량표간상관분석현시，실면（r＝-0．519）、UPDRS Ⅰ（r＝-0．568）、UPDRS Ⅱ（r＝-0．705）、UPDRS Ⅲ（r＝-0．760）、UPDRS Ⅳ（r＝-0．571）、HAMD（r＝-0．669）、HAMA（r＝-0．549）、H-Y분급（r＝-0．694）、PDSS （r＝1．000）、병정（r＝-0．640）、문화정도（r＝0．293）여파금삼병환자생활질량유관（P＜0．01）。다원회귀분석현시，영향EQ-5D 지수인소유 UPDRS Ⅰ、UPDRS Ⅲ、HAMD、HAMA 화 PDSS ，기중 UPDRS Ⅰ、UPDRS Ⅲ、HAMA화 HAMD여EQ-5D지수정부상관（P＜0．05，P＜0．01）；PDSS여EQ-5D지수정정상관（P＜0．01）。결론EQ-5D괄용우파금삼병환자생활질량연구，림상의사재중시파금삼병환자적진단화운동증상치료전제하，환응중시환자수면、초필억욱등비운동증상，진량제고환자생활질량。
Objective To assess the quality of life of patients with Parkinson's disease (PD) with EQ-5D questionnaire and analyze its influencing factors .Methods Fifty-two PD patients were in-cluded in this study .Their quality of life and health status were assessed according to the EQ-5D , UPDRS ,Hoehn-Yahr (H-Y ) stage ,PD sleep scale (PDSS ) ,Hamilton anxiety rating scale (HAMA)amd Hamilton depression rating scale (HAMD) .Results Insomnia (r= -0 .519) , UPDRS Ⅰ - Ⅳ (r= -0 .568 ,r= -0 .705 ,r= -0 .760 ,r= -0 .571) ,HAMD (r= -0 .669) , HAMA (r= -0 .549) ,H-Y stage (r= -0 .694) ,PDSS(r=1 .000) ,disease course (r= -0 .640) and education level (r=0 .293) were related with the quality of life of PD patients ( P<0 .01) . Multivariate regression analysis showed that the factors that influenced the EQ-5D index of PD patients were UPDRS Ⅰ ,UPDRS Ⅲ ,HAMD and PDSS .The UPDRS Ⅰ ,UPDRS Ⅲ ,HAMA and HAMD were negatively related with the EQ-5D index (P<0 .05 ,P<0 .01) ,while the PDSS was positively related with EQ-5D index (P<0 .01) .Conclusion EQ-5D can used in assessment of the quality of life of PD patients .Clinicians should pay their attention not only to the diagnosis of PD patients and treatment of their motor symptoms ,but also to their non-motor symptoms such as sleep ,anxiety and depression in order to improve their quality of life .
