目的:探讨咪达唑仑联合地佐辛运用于无痛胃肠镜检查的麻醉作用。方法:选择行无痛胃肠镜检查的患者80例,随机分为两组,每组40例,每组中各20例分别行胃镜和结肠镜检查。其中Ⅰ组使用咪达唑仑联合哌替啶,Ⅱ组使用咪达唑仑联合地佐辛Ⅱ组。Ⅰ组先缓慢静脉注射咪达唑仑5 mg,后给予哌替啶50 mg静脉注射;Ⅱ组先缓慢静脉注射咪达唑仑5 mg,后给予地佐辛50 mg静脉注射,待睫毛反射消失进行检查操作。记录血氧饱和度、心率变化、睫毛反射消失时间及术毕清醒时间,观察患者术中及术后与麻醉相关的不良反应。结果:两组患者麻醉入睡时间差异无统计学意义(P﹥0.05)。Ⅱ组术毕清醒时间少于Ⅰ组(P﹤0.05)。术中血氧饱和度低于95%、心率小于55次/min发生例数Ⅰ组高于Ⅱ组(P﹤0.05),术中呼吸抑制和术后不适发生率Ⅰ组明显高于Ⅱ组(P﹤0.05),患者苏醒后视觉模拟评分( VAS)优良比例Ⅱ组与Ⅰ组相似( P﹥0.05)。结论:咪达唑仑联合地佐辛用于无痛胃肠镜检查麻醉效果安全确切,优于咪达唑仑联合哌替啶。

목적:탐토미체서륜연합지좌신운용우무통위장경검사적마취작용。방법:선택행무통위장경검사적환자80례,수궤분위량조,매조40례,매조중각20례분별행위경화결장경검사。기중Ⅰ조사용미체서륜연합고체정,Ⅱ조사용미체서륜연합지좌신Ⅱ조。Ⅰ조선완만정맥주사미체서륜5 mg,후급여고체정50 mg정맥주사;Ⅱ조선완만정맥주사미체서륜5 mg,후급여지좌신50 mg정맥주사,대첩모반사소실진행검사조작。기록혈양포화도、심솔변화、첩모반사소실시간급술필청성시간,관찰환자술중급술후여마취상관적불량반응。결과:량조환자마취입수시간차이무통계학의의(P﹥0.05)。Ⅱ조술필청성시간소우Ⅰ조(P﹤0.05)。술중혈양포화도저우95%、심솔소우55차/min발생례수Ⅰ조고우Ⅱ조(P﹤0.05),술중호흡억제화술후불괄발생솔Ⅰ조명현고우Ⅱ조(P﹤0.05),환자소성후시각모의평분( VAS)우량비례Ⅱ조여Ⅰ조상사( P﹥0.05)。결론:미체서륜연합지좌신용우무통위장경검사마취효과안전학절,우우미체서륜연합고체정。
Objective:To investigate the clinical efficacy of intravenous anesthesia using dezocine combined with midazo-lam in patients undergoing painless gastrointestinal endoscope. Methods:Eighty patients who voluntarily required painless gas-trointestinal endoscope were randomly divided into 2 groups(n=40). GroupⅠwas induced by midazolam 5 mg and pethidine 50 mg;Group Ⅱwas induced by midazolam 5 mg and dezocine 50 mg. Before-treatment,under-surgery,weaken-up,pulse oxygen saturation,heart rate were recorded. Patients lost consciousness tim,drug withdrawal eye opening time were recorded. Adverse reactions associated with anesthesia were observed in intraoperative and postoperative. Analgesic effect were observed. Results:Patients lost consciousness time in both groups had no signifcant differences(P﹥0. 05). pulse oxygen saturation,heart rate and awake time in GroupⅡhad better than GroupⅠ(P﹤0. 05). The incidence of intraoperative apnea and postoperative discomfort in groupⅠ were significantly higher than group Ⅱ(P﹤ 0. 05). Analgesic effect in both groups had no signifcant differences(P﹥0. 05). Conclusion:Dezocine combined with midazolam in induced anesthesia has better analgesia effect and safety.
