目的:探讨右美托咪定在减轻胸腔镜手术患者全麻围拔管期应激反应中的作用。方法选择择期于双腔管气管插管全身麻醉下行胸腔镜手术的患者60例(ASAⅠ~Ⅱ),随机分为两组:右美托咪定组(Dex 0.5μg·kg -1,n=30)和对照组(生理盐水10 mL,n =30),记录相应时点患者的心率(HR)、呼吸频率(RR)、收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)及脑电双频谱指数(BIS)值;记录麻醉前、吸痰时、拔管即刻、拔管后2、6 min 血中去甲肾上腺素(NE)、肾上腺素(E)浓度。结果与对照组比较,右美托咪定组于拔管前血压、心率平稳,于导管内吸痰、拔管时及拔管后2、6 min SBP、DBP、MAP、HR 及血中去甲肾上腺素、肾上腺素浓度上升幅度较小,组间比较其差异有统计学意义(P ﹤0.05)。结论右美托咪定可有效减轻全麻胸腔镜手术患者围拔管期应激反应。
目的:探討右美託咪定在減輕胸腔鏡手術患者全痳圍拔管期應激反應中的作用。方法選擇擇期于雙腔管氣管插管全身痳醉下行胸腔鏡手術的患者60例(ASAⅠ~Ⅱ),隨機分為兩組:右美託咪定組(Dex 0.5μg·kg -1,n=30)和對照組(生理鹽水10 mL,n =30),記錄相應時點患者的心率(HR)、呼吸頻率(RR)、收縮壓(SBP)、舒張壓(DBP)、平均動脈壓(MAP)及腦電雙頻譜指數(BIS)值;記錄痳醉前、吸痰時、拔管即刻、拔管後2、6 min 血中去甲腎上腺素(NE)、腎上腺素(E)濃度。結果與對照組比較,右美託咪定組于拔管前血壓、心率平穩,于導管內吸痰、拔管時及拔管後2、6 min SBP、DBP、MAP、HR 及血中去甲腎上腺素、腎上腺素濃度上升幅度較小,組間比較其差異有統計學意義(P ﹤0.05)。結論右美託咪定可有效減輕全痳胸腔鏡手術患者圍拔管期應激反應。
목적:탐토우미탁미정재감경흉강경수술환자전마위발관기응격반응중적작용。방법선택택기우쌍강관기관삽관전신마취하행흉강경수술적환자60례(ASAⅠ~Ⅱ),수궤분위량조:우미탁미정조(Dex 0.5μg·kg -1,n=30)화대조조(생리염수10 mL,n =30),기록상응시점환자적심솔(HR)、호흡빈솔(RR)、수축압(SBP)、서장압(DBP)、평균동맥압(MAP)급뇌전쌍빈보지수(BIS)치;기록마취전、흡담시、발관즉각、발관후2、6 min 혈중거갑신상선소(NE)、신상선소(E)농도。결과여대조조비교,우미탁미정조우발관전혈압、심솔평은,우도관내흡담、발관시급발관후2、6 min SBP、DBP、MAP、HR 급혈중거갑신상선소、신상선소농도상승폭도교소,조간비교기차이유통계학의의(P ﹤0.05)。결론우미탁미정가유효감경전마흉강경수술환자위발관기응격반응。
Objective Discusses the clinical effect of dexmedtomidine at ease the patients' stress respond around the time of tube drawing in thoracoscope surgery with general anesthesia. Methods Choosing 60 cases of surgical patients ac-cepting the double cavity tracheal intubation surgical operation under the condition of general anesthesia(ASA Ⅰ ~ Ⅱlev-el),the 60 cases are divided into two groups:Dex group is the patients who took DEX(Dex 0. 5 μg·kg - 1 ,n = 30)and Comparison group is the comparison group(physiological saline 10 mL,n = 30). Recording the patients' heart rate(HR), respiring rate(RR),systolic pressure(SBP),diastolic pressure(DBP),average arterial pressure(MAP),the index of brain electricity double frequency spectrum before the anesthesia,before using medication,during the corresponding time points. Recording Norepinephrine(NE)in blood,adrenaline(E)density before anesthesia,aspiration of sputum,at the time of de-cannulation,2,6 minutes after the decannulation as well as the grade of vigilance and calmness 2,4,6,10,15 minutes after extubation. Results Compared with comparison group,blood pressure and heart rate of Dex group is steady before decannu-lation,the rising degree of norepinephrine and adrenaline density was less obvious during the period of inner catheter aspira-tion sputum. The same result occurs to SBP,DBP,MAP,HR 2,4 minutes after the decannulation. The level will come back to the normal degree before the decannulation 10 minutes after the operation,there being a statistic significance comparing the research results between these 2 groups(P ﹤ 0. 05). Conclusion Dexmedtomidine can ease the patients' stress respond obviously around the time of tube drawing in thoracoscope surgery with general anesthesia.
