目的:探讨浅低温治疗急性呼吸窘迫综合症( ARDS)是否影响组织脏器功能,探讨临床应用安全性。方法68例ARDS行浅低温治疗,观察治疗前后动态心电图、肝肾功能、凝血功能变化、血浆肌钙蛋白浓度等,评估浅低温治疗对机体氧合情况、心律失常、组织器官功能情况有无不良影响。结果与浅低温治疗前相比,动态血气分析提示氧分压(PaO2)、氧合指数逐步升高,肺泡-动脉氧分压差下降(P(A-a)O2)(P<0.05),肺功能改善;动态心电图检查未发现室速、室颤等恶性心律失常,血浆肌钙蛋白浓度治疗前后无差异(P>0.05),肝肾、凝血功能均无统计学差异(P>0.05)。结论浅低温治疗ARDS不影响重要组织器官功能,临床应用是安全、有效的。

목적:탐토천저온치료급성호흡군박종합증( ARDS)시부영향조직장기공능,탐토림상응용안전성。방법68례ARDS행천저온치료,관찰치료전후동태심전도、간신공능、응혈공능변화、혈장기개단백농도등,평고천저온치료대궤체양합정황、심률실상、조직기관공능정황유무불량영향。결과여천저온치료전상비,동태혈기분석제시양분압(PaO2)、양합지수축보승고,폐포-동맥양분압차하강(P(A-a)O2)(P<0.05),폐공능개선;동태심전도검사미발현실속、실전등악성심률실상,혈장기개단백농도치료전후무차이(P>0.05),간신、응혈공능균무통계학차이(P>0.05)。결론천저온치료ARDS불영향중요조직기관공능,림상응용시안전、유효적。
Objective To explore the effect and safety of mild hypothermia in the treatment of acute respira-tory distress syndrome ( ARDS) . Methods 68 cases of ARDS were given mild hypothermia treatment to observe dy-namic electrocardiogram, liver and kidney function, blood coagulation function changes, and plasma troponin concen-trations. The effect of mild hypothermia treatment on body oxygenation, arrhythmia, tissues and organs function con-ditions was assessed. Results After the treatment, the dynamic blood gas analysis suggested oxygen partial pressure ( PaO2 ) and oxygenation index gradually increased, alveolar-arterial oxygen partial pressure difference decreased ( P (A-A)O2) (P<0. 05), and lung function improved. Dynamic electrocardiogram inspection found no ventricular tachycardia or ventricular fibrillation. There was no significant difference in plasma troponin concentration, liver, kidney and blood coagulation function before and after the treatment ( P>0. 05 ) . Conclusion Mild hypothermia treatment of ARDS does not affect important tissues and organs function in the treatment of ARDS, which is a safe and effective application.
