目的探讨肝叶切除结合囊肿开窗术治疗巨大多囊肝的疗效。方法回顾性分析近期1例巨大多囊肝的临床、影像学资料及手术疗效。结果采用肝右三叶切除加囊肿开窗去顶术治疗，该治疗切除了病变的肝脏，充分解除了囊肿压迫，剩余肝实质内囊肿开窗去顶，减少了残存的肝囊肿，手术效果好，临床症状消失，术后并发症少，恢复满意，顺利出院。结论肝叶切除结合囊肿开窗术治疗巨大多囊肝安全有效，术后并发症少，可作为该类疾病的首选治疗方法。

Conclusion The combination of cyst fenestration hepatectomy treating polycystic liver huge safe and ef ective, fewer complications, can be used as the preferred method of treatment of such diseases.
