目的研究动脉瘤性蛛网膜下腔出血(aSAH)患者血清中可溶性细胞间黏附分-1(sICAM-1)含量动态变化规律,分析其与脑血管痉挛(CVS)发生的时项、程度的相关性。方法对40例动脉瘤性蛛网膜下腔出血患者(实验组)发病后1~3d(术前)、7~9d、11~13d血清中sICAM-1(用ELISA法进行检测)进行动态检测,用TCD检测大脑中动脉血流速度(VMCA )。结果动脉瘤性蛛网膜下腔出血患者血清中sICAM-1含量明显高于对照组,尤以发病后7~9d显著;患者脑血管痉挛程度越重,血清中sICAM-1含量越高。结论动脉瘤性蛛网膜下腔出血患者血清中的slCAM-1含量高低可以反映患者脑血管痉挛病情的严重程度,可以作为动脉瘤性蛛网膜下腔出血患者发病后脑血管痉挛病情判断、指导临床治疗及预后判断的有效指标。

Objective To study the relationship between the serum concentration of sICAM-1with subarachnoid hemorrhage (aSAH) and the change of symptom,analysis its and cerebral vasospasm (CVS) of items,the degree of correlation.Methods Dynamical changes of the serum concentration of sICAM-1 during dif er days were observed in 40 cases with aSAH , TCD was used to measure VMCA ,and control group for the same period to our healthy adult physical examination of 20 cases. Results Al the indexes in 1~3 days, 7~9 days, 11~13 after aSAH were significantly dif erent ccmpared with those of the controls. The indexes of 7~9 days after aSAH were most significant;The higher the Hunt-Hess classification and grade of Fisher, the higher the serum content of sICAM-1; Degree of patients with cerebral vasospasm is heavier, the higher the serum content of sICAM-1.Conclusion This findings suggest that there be a positive correlation between the serum concentration of sICAM-1 and the change of symptom CVS.
