[目的]探讨胃癌根治术后两种镇痛泵的应用效果。[方法]将80例行胃癌根治术的胃癌病人采用随机数字表法分为对照组和观察组各40例,对照组病人术后使用硬膜外导管镇痛泵镇痛(PCEA),观察组病人术后使用静脉镇痛泵镇痛(PCIA),均给予相应的临床观察和护理,采用视觉模拟评分法(VAS)评定疼痛程度,比较两组病人的术后疼痛情况、术后并发症情况、护理满意度情况。[结果]观察组病人术后疼痛程度明显轻于对照组、术后 VAS 评分及并发症发生率均明显低于对照组、护理满意度明显高于对照组,经比较差异均有统计学意义(P<0.05)。[结论]PCEA 与 PCIA均是胃癌根治术的有效镇痛方法,可明显减轻病人的术后疼痛,PCIA的镇痛效果更好,术后并发症少且安全性高。
[목적]탐토위암근치술후량충진통빙적응용효과。[방법]장80례행위암근치술적위암병인채용수궤수자표법분위대조조화관찰조각40례,대조조병인술후사용경막외도관진통빙진통(PCEA),관찰조병인술후사용정맥진통빙진통(PCIA),균급여상응적림상관찰화호리,채용시각모의평분법(VAS)평정동통정도,비교량조병인적술후동통정황、술후병발증정황、호리만의도정황。[결과]관찰조병인술후동통정도명현경우대조조、술후 VAS 평분급병발증발생솔균명현저우대조조、호리만의도명현고우대조조,경비교차이균유통계학의의(P<0.05)。[결론]PCEA 여 PCIA균시위암근치술적유효진통방법,가명현감경병인적술후동통,PCIA적진통효과경호,술후병발증소차안전성고。
Obj ective:To probe into the application effect of two kinds of analgesic pump for gastric cancer patients after radical resection.Meth-ods:A total of 80 gastric cancer patients undergoing radical resection were randomly divided into control group and observation group,40 cases in each.Patients in control group received postoperative catheter epidural analgesia (PCEA).Patients in observation group used postoperative cath-eter intravenous analgesia (PCIA).All patients were given corresponding clinical observation and nursing care.Pain was evaluated by using visual analogue scale (VAS ).Postoperative pain,postoperative complications and nursing satisfaction were compared between two groups.Results:Postoperative pain in observation group was significantly better than that in control group.VAS score and incidence of postoperative complications in observation group were significantly lower than that in control group. Nursing satisfaction in observation group was significantly higher than that in control group.The differences were statistically significant (P<0.05). Conclusion:PCEA and PCIA are the effective analgesia methods after radical resection of gastric cancer patients,which can significantly reduce postoperative pain.PCIA has better effect and has less postoperative com-plications and higher safety.
