目的：探讨乌司他丁对毒蛇咬伤合并急性肾功能损伤患者的肾功能和血TNF-α、IL-6的影响及疗效。方法将45例毒蛇咬伤并急性肾功能损伤患者随机分为治疗组（n＝25）和对照组（n＝20），两组均给予综合治疗，治疗组在综合治疗的基础上静脉注射乌司他丁。观察治疗后两组血尿素氮、血肌酐、TNF-α和IL-6水平变化，同时观察两组的治愈率和平均进入多尿期时间。结果治疗组的血尿素氮、血肌酐、TNF-α和IL-6水平均较对照组下降明显（P＜0.05），同时治疗组平均进入多尿期时间也短于对照组（P＜0.05），但两组的治愈率无统计学差异（P＞0.05）。结论乌司他丁能改善毒蛇咬伤合并急性肾功能损伤患者的肾功能和 TNF-α和IL-6指标，明显缩短病程，减少住院时间。
목적：탐토오사타정대독사교상합병급성신공능손상환자적신공능화혈TNF-α、IL-6적영향급료효。방법장45례독사교상병급성신공능손상환자수궤분위치료조（n＝25）화대조조（n＝20），량조균급여종합치료，치료조재종합치료적기출상정맥주사오사타정。관찰치료후량조혈뇨소담、혈기항、TNF-α화IL-6수평변화，동시관찰량조적치유솔화평균진입다뇨기시간。결과치료조적혈뇨소담、혈기항、TNF-α화IL-6수평균교대조조하강명현（P＜0.05），동시치료조평균진입다뇨기시간야단우대조조（P＜0.05），단량조적치유솔무통계학차이（P＞0.05）。결론오사타정능개선독사교상합병급성신공능손상환자적신공능화 TNF-α화IL-6지표，명현축단병정，감소주원시간。
Objective To investigate the change of kidney function and serum TNF-α and IL-6 levels in patients bitten by venomous snake and acute kidney injury who treated with ulinastatin . Methods Forty-five patients were analyzed and dived into two groups ,treatment group (n= 25) and control group (n= 20) .Patients derived from treatment group were treated with ulinastatin as well as conventional treatment .The observed indexes in this study included serum levels of blood uria nitrogen (BUN) ,creatinine (SCr) ,TNF-α,and IL-6 ;the rate of recovery and mean time of entering polyuria stage were observed as well . Results Serum levels of BUN ,SCr ,TNF-α,and IL-6 were significantly more decreased in treatment group than control group (P<0.05) .The mean time of entering polyuria stage was more shorter in treatment group as well(P<0.05) .However ,the rate of recovery of both two groups was not statistic difference(P>0.05) . Conclusion Serum levels of BUN ,SCr ,TNF-α,and IL-6 in pa-tients bitten by venomous snake and acute kidney injury who treated with ulinastatin are improved significantly .
