目的：探讨子宫动脉栓塞联合B超下清宫术在治疗剖宫产后子宫瘢痕妊娠的效果。方法收集本院26例子宫瘢痕妊娠患者的临床资料，分析子宫动脉栓塞联合 B 超监测下清宫治疗子宫瘢痕妊娠的疗效。结果术后血hCG恢复正常水平时间为（43.6±12.1）d；瘢痕妊娠组织B超监测消退的时间为（33.4±4.3）d，住院时间为（12.3±2.5）d；在B超监测下均顺利清宫，术中阴道出血量为10～150 ml，未出现大出血、子宫穿孔等并发症；5例患者2次清宫后治愈，治愈率100％。结论 CSP治疗应根据患者情况个性化治疗；经子宫动脉栓塞联合B超下清宫术创伤小、恢复快，是瘢痕妊娠预防大出血的安全、有效的治疗方法之一。
목적：탐토자궁동맥전새연합B초하청궁술재치료부궁산후자궁반흔임신적효과。방법수집본원26례자궁반흔임신환자적림상자료，분석자궁동맥전새연합 B 초감측하청궁치료자궁반흔임신적료효。결과술후혈hCG회복정상수평시간위（43.6±12.1）d；반흔임신조직B초감측소퇴적시간위（33.4±4.3）d，주원시간위（12.3±2.5）d；재B초감측하균순리청궁，술중음도출혈량위10～150 ml，미출현대출혈、자궁천공등병발증；5례환자2차청궁후치유，치유솔100％。결론 CSP치료응근거환자정황개성화치료；경자궁동맥전새연합B초하청궁술창상소、회복쾌，시반흔임신예방대출혈적안전、유효적치료방법지일。
Objective Toassesstheefficacyofuterinearteriesembolizationcombinedwithuterinecurettageunder ultrasonography for the treatment of cesarean scar pregnancies (CSP ). Methods Twenty-six women with CSP were identified between May 2010 and May 2013.All of the patients underwent uterine arteries embolization combined with uterine curettage under ultrasonography. Results All the patients were successfully treated.The mean time until normalization of serum human chorionic gonadotrophin was 43.6 days,and the mean time until CSP mass disappearance was 33.4 days.The mean hospitalization time was 12.3 days.The complications were mainly fever and pain.All patients recovered their normal menstruation at follow-up. Conclusion Patients with CSP should be treated individually. Bilateral uterine artery embolization combined with uterine curettage under ultrasonography appears to be a safe and effective treatment for CSP.
