目的:探讨胸透、胸片及高分辨CT在不典型气管支气管异物诊断中的价值。方法:对45例经纤维支气管内镜诊治的不典型气管支气管异物的胸透、胸片及高分辨CT检查的结果进行对比。结果:45例经纤维支气管内镜验证:胸透阳性率52.94%,假阳性率17.65%,假阴性率11.76%,不确诊17.65%;胸片阳性率53.85%,假阳性率15.38%,假阴性率15.38%,不确诊15.38%;高分辨CT阳性率100.00%,假阳性和假阴性均为0。胸透与胸片对不典型气管支气管异物诊断阳性率差异无统计学意义(P>0.05),但高分辨CT及重建的诊断阳性率均高于胸透和胸片阳性率(P<0.05)。结论:胸部透视、胸片简单、经济,但对不典型气管支气管异物的诊断敏感性不高。高分辨CT对不典型气管支气管异物的诊断具有非常重要的价值,并对临床具有指导意义,是不典型气管支气管异物无创检查的可靠方法。

목적:탐토흉투、흉편급고분변CT재불전형기관지기관이물진단중적개치。방법:대45례경섬유지기관내경진치적불전형기관지기관이물적흉투、흉편급고분변CT검사적결과진행대비。결과:45례경섬유지기관내경험증:흉투양성솔52.94%,가양성솔17.65%,가음성솔11.76%,불학진17.65%;흉편양성솔53.85%,가양성솔15.38%,가음성솔15.38%,불학진15.38%;고분변CT양성솔100.00%,가양성화가음성균위0。흉투여흉편대불전형기관지기관이물진단양성솔차이무통계학의의(P>0.05),단고분변CT급중건적진단양성솔균고우흉투화흉편양성솔(P<0.05)。결론:흉부투시、흉편간단、경제,단대불전형기관지기관이물적진단민감성불고。고분변CT대불전형기관지기관이물적진단구유비상중요적개치,병대림상구유지도의의,시불전형기관지기관이물무창검사적가고방법。
Objective:To investigate the clinical value of chest fluoroscopy, chest radiography and high-resolution computed tomography( CT) in the diagnosis of atypical foreign body in trachea and bronchus. Methods:The data of chest fluoroscopy, chest radiography and high-resolution CT in 45 cases with atypical foreign body in trachea and bronchus diagnosed by fiberbronchoscopy were analyzed and compared. Results:The positive rate,false positive rate,false negative rate and misdiagnosis rate of chest fluoroscopy were 52. 94%,17. 65%,11. 76% and 17. 65%,respectively. The positive rate,false positive rate,false negative rate and misdiagnosis rate of chest radiography were 53. 85%,15. 38%,15. 38% and 15. 38%,respectively. The positive rate of CT was 100. 00%,its false positive and negative rates were 0. 00% and 0. 00%,respectively. The difference of the positive rate in diagnosing the atypical foreign body in trachea and bronchus between chest fluoroscopy and chest radiography was not statistically significant(P>0. 05),but the diagnosic positive rate of high-resolation CT and reconstruction were higer than the fluoroscopy and radiography(P<0. 05). Conclusions:The sensibilities of chest fluoroscopy and radiography in diagnosing the atypical foreign body in trachea and bronchus are low, but CT detection is high. The CT detection is a reliable and noninvasive method for demonstrating the typical foreign body in trachea and bronchus.
