目的:观察中药灌肠对慢性肾脏疾病( CKD)患者血清甲状旁腺素( PTH)的清除作用。方法将88例CKD患者随机分为2组,对照组45例予常规治疗,治疗组43例在对照组治疗基础上予中药灌肠治疗,2组均治疗12周。比较2组中医证候疗效,观察2组治疗前后血清肌酐( Cr )、尿素氮( BUN )及PTH变化。结果2组中医证候总有效率比较差异有统计学意义(P<0.05),治疗组中医证候疗效优于对照组。治疗组治疗后Cr、BUN及PTH均较本组治疗前、对照组治疗后降低( P<0.05)。结论中药灌肠可有效改善CKD患者肾功能,降低Cr、BUN等小分子尿毒症毒素的浓度,更可以降低血清PTH浓度,可有效配合血液透析滤过等肾脏替代治疗。

목적:관찰중약관장대만성신장질병( CKD)환자혈청갑상방선소( PTH)적청제작용。방법장88례CKD환자수궤분위2조,대조조45례여상규치료,치료조43례재대조조치료기출상여중약관장치료,2조균치료12주。비교2조중의증후료효,관찰2조치료전후혈청기항( Cr )、뇨소담( BUN )급PTH변화。결과2조중의증후총유효솔비교차이유통계학의의(P<0.05),치료조중의증후료효우우대조조。치료조치료후Cr、BUN급PTH균교본조치료전、대조조치료후강저( P<0.05)。결론중약관장가유효개선CKD환자신공능,강저Cr、BUN등소분자뇨독증독소적농도,경가이강저혈청PTH농도,가유효배합혈액투석려과등신장체대치료。
Objective To find out the effect of Chinese medicine enema on PTH clear in blood for patients of chronic kidney disease ( CKD) .Methods 88 CKD patients were randomly divided into two groups .Control group ( n =43 ) received routine therapy .45 patients in treatment group were treated by Chinese medicine enema .The changes of Cr ,BUN and PTH before and after treatment were observed in two groups and the effect of traditional Chi -nese medicine syndrome was compared after 12 weeks of treatment .Results The self quasi square could improve the renal function of the patients .Further more it could not only reduce the concentration in blood of micromolecular ure-mic toxins such as SCr ,BUN,but also reduce the concentration in blood of middle and macromolecular uremic toxins such as PTH.Conclusion Chinese medicine enema can significantly improve renal function of CKD patients ,reduce the concentration of Cr ,BUN and other small molecule uremic toxins and can effectively coordinate hemodiafiltration and other renal replacement therapy .
