目的:探讨脑梗死后癫痫患者免疫球蛋白、补体的变化及意义。方法抽取我院2012-2013年36例脑梗死后癫痫患者、42例脑梗死史无癫痫患者及48例正常对照者静脉血各2mL,采用散射比浊法检测免疫球蛋白和补体含量,并进行分析。结果对照组、脑梗死无癫痫组、脑梗死后癫痫组血清IgA水平逐渐下降,差异有统计学意义(P<0.05)。血清IgG、IgM、C3、C4浓度与对照组比较差异无统计学意义(P>0.05)。多因素Logistic回归分析显示,梗死部位、血清IgA水平与脑梗死后癫痫发生有关。脑梗后癫痫患者血清IgA浓度ROC曲线下面积AUC=0.821。结论IgA在脑梗死后癫痫发生机制中起作用。

목적:탐토뇌경사후전간환자면역구단백、보체적변화급의의。방법추취아원2012-2013년36례뇌경사후전간환자、42례뇌경사사무전간환자급48례정상대조자정맥혈각2mL,채용산사비탁법검측면역구단백화보체함량,병진행분석。결과대조조、뇌경사무전간조、뇌경사후전간조혈청IgA수평축점하강,차이유통계학의의(P<0.05)。혈청IgG、IgM、C3、C4농도여대조조비교차이무통계학의의(P>0.05)。다인소Logistic회귀분석현시,경사부위、혈청IgA수평여뇌경사후전간발생유관。뇌경후전간환자혈청IgA농도ROC곡선하면적AUC=0.821。결론IgA재뇌경사후전간발생궤제중기작용。
Objective To discuss the change of the levels of serum IgG ,IgA ,IgM and complement C3 ,C4 in patients with epilepsy after cerebral infarction and study their clinical significance.Methods Venous blood of 36 cases with epilepsy af-ter cerebral infarction ,42 cases without epilepsy after cerebral infarction and 48 cases in normal control group were extracted . The concentration of IgG ,IgA ,IgM and complementC3 ,C4 was measured with scattering turbidimetry and was analyzed. Re-sults The concentration of IgA gradually declined between control group ,non-epilepsy after cerebral infarction and epilepsy af-ter cerebral infarction (P<0.05 for all) ,the levels of IgG ,IgM ,C3 ,C4 of patients with epilepsy after cerebral infarction and normal control had no significant difference. Multivariate logistic regression showed that cerebral infarction location and the concentration of IgA were associated with the development of epilepsy. The AUC area of the concentration of IgA under ROC curve=0.821. Conclusion IgA plays an important role in the pathogenesis of epilepsy after cerebral infarction.
