目的:对维持性血液透析患者使用不同的血液净化方式,分析对残余肾功能(rrf)的影响。方法随机抽取我院进行常规血液透析的患者共44例,分为低通量血液透析组(lfHd组14例)、高通量血液透析组(HfHd组15例)及血液透析联合血液灌流组(Hd+HP组15例)。在患者入选时、治疗6个月后采集所有患者的血尿标本,对患者的相关指标进行检测。结果入选时三组患者之间的残余肾功能、血脂、血磷、β2微球蛋白没有统计学意义上的差异。经治疗6个月后患者的残余肾功能较入选时明显降低,但lfHd组残余肾功能下降率高于HfHd组和Hd+HP组。治疗后患者的血脂、血磷、β2微球蛋白,HfHd组和Hd+HP组明显低于lfHd组,HfHd组和Hd+HP组之间没有统计学意义上的差异。结论患者实施血液透析过程中,残余肾功能均有所下降,但HfHd组、Hd+HP组与lfHd组比较,残余肾功能下降率较低,同时与lfHd组相比,能够更好的降低血脂、血磷、β2微球蛋白水平。

Objectiveto analyze the impact of different blood purification methods on residual renal function in maintenance hemodialysis patients. Methods 44 patients treated with hemodialysis were randomly selected in our hospital. they were divided into low-flux hemodialysis group (14 cases of lfHd group), high-flux hemodialysis group (15cases of HfHd group) and blood hemodialysis combined with hemoperfusion group (15 cases of Hd + HP group). When patients were enrolled, all patients’ urine specimens were collected after treatment for 6 months and the relevant indicators of patients were detected. Results There were no statistically signiifcant differences in residual renal function, level of lipid, phosphorus and β2-microglobulin of patients among the three groups before treatment. After treated for 6 months, patients’ residual renal function was signiifcantly lower compared to that before treatment, but the decline degree of residual renal function in LFHD group was higher than that in HfHd group and Hd + HP group. after treatment, the patient's blood level of lipids, phosphate and β2-microglobulin in HFHD group and HD + HP group was signiifcantly lower than that in LFHD group, but there was no statistically signiifcant difference between the HFHD group and the HD + HP group. Conclusion After hemodialysis, the residual renal function of patients all declined. But compared with HfHd group and Hd + HP group, there was a lower rate of residual renal function decline in lfHd group, while compared with lfHd group, the treatment of HfHd and Hd + HP can reduce the level of blood lipids, phosphorus and β2-microglobulin levels.
