目的探讨肝细胞癌(HCC)经导管肝动脉化疗栓塞术(TACE)治疗后缺氧诱导因子(HIF)-1α与Foxp3的表达及意义。方法收集2002年1至4月,在哈尔滨医科大学附属第三医院介入科TACE术后行Ⅱ期手术切除的HCC标本31例(TACE治疗组)和未经任何治疗直接手术切除的HCC组织标本37例(单纯手术组)。采用免疫组织化学SP法,检测两组标本中Foxp3及HIF-1α的表达情况。采用χ2检验,以P<0.05为差异有统计学意义。结果 HIF-1α和Foxp3在TACE治疗组的阳性表达倒数和百分率分别为28例(90.3%),27例(87.1%),均显著高于单纯手术组23例(62.2%),22例(60.0%),统计学分析χ2=6.785、6.399,P均<0.05。TACE组HIF-1α与Foxp3表达呈正相关(r=0.537,P=0.006)。结论 TACE能明显上调HIF-1α和Foxp3的表达,从而增强了肿瘤局部的免疫抑制功能。这可能是HCC经TACE后残癌复发、转移的原因之一。

Objective To investigate the expression and significance of HIF-1α and Foxp3 in the residual tumor tissues of hepatocellular carcinoma(HCC) after transcatheter arterial chemoembolization(TACE). Methods The surgical specimens of HCC after TACE were colleted in 31 cases (TACE group), and surgical specimens of HCC without having received any other treatment were obtained in 37 cases(pure surgery group).The expression of HIF-1αand Foxp3 in the residual tumor tissues was determined by using immunohistochemical SP method. Results The positive expression of HIF-1αand Foxp3 in TACE group(90.34%, 87.09%)were higher than those in direct surgery group (62.16%, 59.50%, P=0.008, 0.012). For the residual cancerous tissues of TACE group, the positive expression of HIF-1αwas positively correlated with Foxp3 (r=0.537, P=0.006). Conclusions Signiifcant overexpression of HIF-1αand Foxp3 is found in patients with HCC after TACE, and enhances the immune suppression of local tumor. which may be one of causes of recurrence and metastasis of HCC after TACE.
