目的:探讨肥胖男性瘦素(Leptin)水平与男性不育相关各精液参数以及内分泌激素的相关性。方法从上海交通大学医学院附属仁济医院男科门诊就诊患者中选取30例,设为肥胖组;另选取正常健康男性10例为对照组。对两组都进行精液参数分析、内分泌激素检测和血清Leptin水平检测。结果对照组精子密度明显高于肥胖组(P<0.01),对照组A级活动精子和B级活动精子百分比都明显高于肥胖组(P<0.01)。对照组促卵泡生成素(FSH)、促黄体激素(LH)和睾酮(T)水平明显高于肥胖组(P<0.05)。对照组血清Leptin水平明显低于肥胖组(P<0.01)。血清Leptin水平与精子密度之间有显著负相关性(P<0.05)血清Leptin水平与FSH、LH、T水平之间均有显著负相关性(P<0.05)。结论血清Leptin水平与精液各项参数以及内分泌激素有非常密切的关系,可能抑制内分泌激素,影响精子生成,最终导致精子数量和活力的下降。

목적:탐토비반남성수소(Leptin)수평여남성불육상관각정액삼수이급내분비격소적상관성。방법종상해교통대학의학원부속인제의원남과문진취진환자중선취30례,설위비반조;령선취정상건강남성10례위대조조。대량조도진행정액삼수분석、내분비격소검측화혈청Leptin수평검측。결과대조조정자밀도명현고우비반조(P<0.01),대조조A급활동정자화B급활동정자백분비도명현고우비반조(P<0.01)。대조조촉란포생성소(FSH)、촉황체격소(LH)화고동(T)수평명현고우비반조(P<0.05)。대조조혈청Leptin수평명현저우비반조(P<0.01)。혈청Leptin수평여정자밀도지간유현저부상관성(P<0.05)혈청Leptin수평여FSH、LH、T수평지간균유현저부상관성(P<0.05)。결론혈청Leptin수평여정액각항삼수이급내분비격소유비상밀절적관계,가능억제내분비격소,영향정자생성,최종도치정자수량화활력적하강。
Objective To explore the correlation of the serum Leptin level of obsen male and semen parameters, as well as endocrine hormones level related to male sterility. Methods Total of 30 patients (the obesity group) and 10 healthy men (the control group) were enrolled in the study. The CASA was used to analyze semen quality of the patients. The levels of serum endocrine hormone were measured by Chemiluminescence, and the level of serum Leptin was detected by ELISA. Results The sperm concentration and a+b grade sperm motility percentage in the control group were significantly higher than that in the obesity group (P<0.01). Levels of FSH, LH and T in the control group were significantly lower than those in the obesity group(P<0.05). The level of serum Leptin in the control group was significantly higher than that in the obesity group(P<0.01). The level of serum Leptin was significantly negatively correlated with sperm concentration and the levels of FSH、LH and T. Conclusion There are significant correlation between serum Leptin level and sperm parameters, as well as endocrine hormones level. Leptin may inhibit endocrine hormones and affect sperm production, then resulting in the decline of sperm number and motility.
