目的检测系统性红斑狼疮( SLE)患者和健康者外周血CD19+ B细胞中B淋巴细胞诱导成熟蛋白-1(Blimp-1) mRNA的表达,并探讨Blimp-1在SLE的临床意义。方法应用实时荧光定量PCR技术检测40例SLE患者和20例健康者外周血CD19+ B细胞中Blimp-1 mRNA的表达水平,应用流式细胞术检测外周血中CD19-CD138+浆细胞/CD19+B细胞比例、CD19+CD27+记忆性B细胞/CD19+ B细胞比例,分析SLE患者Blimp-1表达与B细胞亚群及临床相关指标的相关性。结果① SLE 组 CD19+ B 细胞中 Blimp-1 mRNA的表达水平高于正常对照组( P<0.05)、活动组高于稳定组(P<0.05);② SLE组CD19-CD138+浆细胞/CD19+B细胞比例显著高于正常对照组( P<0.01)、活动组高于稳定组( P <0.05);SLE 组与正常对照组外周血中 CD19+CD27-记忆性B细胞/CD19+ B细胞比例差异无统计学意义( P>0.05);③SLE患者Blimp-1表达与CD19-CD138+浆细胞/CD19+ B细胞比例呈正相关性(rs =0.336,P=0.034),与CD19+CD27+记忆性 B细胞/CD19+ B细胞比例无显著相关性(rs =-0.271,P=0.116),与抗ds-DNA抗体、免疫球蛋白IgG呈正相关性( rs =0.493,P =0.006;rs =0.527,P=0.004),与抗核小体抗体、抗 C1q 抗体呈正相关性( rs =0.452,P =0.040;rs =0.522,P =0.015),与白细胞数量、血小板数量呈负相关性(rs =-0.367,P=0.046;rs =-0.386, P =0.035)。结论 Blimp-1在SLE患者B细胞中表达升高,与浆细胞明显相关,提示Blimp-1可能通过促进B细胞分化而参与SLE的发病。

목적검측계통성홍반랑창( SLE)환자화건강자외주혈CD19+ B세포중B림파세포유도성숙단백-1(Blimp-1) mRNA적표체,병탐토Blimp-1재SLE적림상의의。방법응용실시형광정량PCR기술검측40례SLE환자화20례건강자외주혈CD19+ B세포중Blimp-1 mRNA적표체수평,응용류식세포술검측외주혈중CD19-CD138+장세포/CD19+B세포비례、CD19+CD27+기억성B세포/CD19+ B세포비례,분석SLE환자Blimp-1표체여B세포아군급림상상관지표적상관성。결과① SLE 조 CD19+ B 세포중 Blimp-1 mRNA적표체수평고우정상대조조( P<0.05)、활동조고우은정조(P<0.05);② SLE조CD19-CD138+장세포/CD19+B세포비례현저고우정상대조조( P<0.01)、활동조고우은정조( P <0.05);SLE 조여정상대조조외주혈중 CD19+CD27-기억성B세포/CD19+ B세포비례차이무통계학의의( P>0.05);③SLE환자Blimp-1표체여CD19-CD138+장세포/CD19+ B세포비례정정상관성(rs =0.336,P=0.034),여CD19+CD27+기억성 B세포/CD19+ B세포비례무현저상관성(rs =-0.271,P=0.116),여항ds-DNA항체、면역구단백IgG정정상관성( rs =0.493,P =0.006;rs =0.527,P=0.004),여항핵소체항체、항 C1q 항체정정상관성( rs =0.452,P =0.040;rs =0.522,P =0.015),여백세포수량、혈소판수량정부상관성(rs =-0.367,P=0.046;rs =-0.386, P =0.035)。결론 Blimp-1재SLE환자B세포중표체승고,여장세포명현상관,제시Blimp-1가능통과촉진B세포분화이삼여SLE적발병。
Objective To detect the mRNA expression of B lymphocyte induced maturation protein-1 ( Blimp-1 ) in peripheral blood CD19 + B cells from patients with systemic lupus erythromatosus( SLE) and healthy people,and ex-plore its clinical significance. Methods Expression level of Blimp-1 mRNA in peripheral blood CD19 + B cells was determined by real-time PCR in 40 SLE patients and 20 healthy controls. The ratio of CD19 -CD138 + plasma cells and CD19 + B cells and the ratio of CD19 +CD27 -memory B cells and CD19 + Bcells were studied by flow cytometry in peripheral blood from two groups. Results ① The expression level of Blimp-1 mRNA in peripheral blood CD19 +B cells from SLE patients group was higher than that in normal control group(P<0.05),and the expression level of Blimp-1 in active SLE patients group was higher than that in inactive SLE patients group ( P<0.05 ) . ②The CD19 -CD138 + plasma cells/CD19 + B cells ratio was increased in SLE patients group compared with healthy control group(P<0.01),and the CD19 -CD138 + plasma cells/CD19 + B cells ratio in active SLE patients group was more increased than that in inactive SLE patients group(P<0.05). However,the CD19 +CD27 - memory B cells/CD19 + Bcells ratio was not significantly different between SLE patients group and normal control group( P>0.05 ) .③The expression level of Blimp-1 was positively correlated with the CD19 -CD138 + plasma cells/CD19 + B cells ratio(rs=0.336,P=0.034),not correlated with the CD19 +CD27 -memory B cells/CD19 + B cells ratio(rs=-0.271,P=0.116),positively correlated with anti-dsDNA antibody and immunoglobulin G(rs =0.493,P =0.006;rs =0.527,P =0.004),positive correlated with anti-nucleosome antibody and anti-C1q antibody(rs =0.452,P =0.040;rs=0.522,P =0.015),negatively correlated with white blood cell count and platelet count(rs= -0.367,P=0.046;rs = -0.386,P=0.035). Conclusion Blimp-1 expression is up-regulated in SLE pa-tients and significantly correlated with plasma cells,which suggests that Blimp-1 might be involved in the pathogene-sis of SLE by promoting plasma cell differentiation.
