目的:探讨帕金森病( PD)患者血浆同型半胱氨酸( Hcy)、叶酸、维生素B12、丙二醛( MDA)浓度与病情严重程度、病程进展及认知和情绪的关系。方法通过检测PD患者的Hcy、叶酸、维生素B12、丙二醛浓度,并应用简明智力量表( MMSE)测查PD患者的认知功能、汉密顿抑郁量表(HAMD)测查PD患者的情绪,并结合患者的临床特征进行分析。结果帕金森患者中Hcy升高占57%。 Hcy升高组与 Hcy正常组比较,叶酸和维生素B12低,MDA增高,H-Y分期评分高,MMSE低,HAMD增高。结论 Hcy增高的PD患者病情较严重,更易出现认知功能受损和抑郁情绪,叶酸、维生素B12缺乏是Hcy增高的营养代谢因素,Hcy增高可能诱导氧化应激而导致多巴胺能神经元损害。补充叶酸和维生素 B12降低PD患者血浆Hcy水平,可能延缓PD疾病进展,改善预后。

목적:탐토파금삼병( PD)환자혈장동형반광안산( Hcy)、협산、유생소B12、병이철( MDA)농도여병정엄중정도、병정진전급인지화정서적관계。방법통과검측PD환자적Hcy、협산、유생소B12、병이철농도,병응용간명지역량표( MMSE)측사PD환자적인지공능、한밀돈억욱량표(HAMD)측사PD환자적정서,병결합환자적림상특정진행분석。결과파금삼환자중Hcy승고점57%。 Hcy승고조여 Hcy정상조비교,협산화유생소B12저,MDA증고,H-Y분기평분고,MMSE저,HAMD증고。결론 Hcy증고적PD환자병정교엄중,경역출현인지공능수손화억욱정서,협산、유생소B12결핍시Hcy증고적영양대사인소,Hcy증고가능유도양화응격이도치다파알능신경원손해。보충협산화유생소 B12강저PD환자혈장Hcy수평,가능연완PD질병진전,개선예후。
Objective To explore the relationship between the concentration of homocysteine (Hcy) , folic acid, vitamin B12, mal-onialdehyde(MDA) and the levels of progression, cognition, emotion of Parkinson′s disease(PD) patients.Methods The concentrations of Hcy, folic acid, vitamin B12, MDA were detected and cognitive function was evaluated by Mini-mental State Examination (MMSE) and e-motion was measured by Hamilton Depression scale ( HAMD) in PD.The clinical characteristics of the patients were analyzed .Results The level of Hcy of 57%PD was increased .The levels of folic acid , vitamin B12 in Hcy increasing group were decreased , the level of MDA was increased, H-Y staging scores were higher , the score of MMSE was lower and the score of HAMD was higher compared with those of Hcy norma group.Conclusions The high Hcy level might enhance oxidative stress and induce the dopaminergic neuron damage .The supple-ments of folic acid and vitamin B 12 are helpful to reduce the level of Hcy , delay progression and improve prognosis in PD patients .
