目的:探讨高流量湿化氧疗系统对气管切开患者医院获得性肺炎的影响。方法:将75例均已脱离呼吸机气管切开患者按照随机数字表法分为试验组和对照组,试验组45例,对照组30例。对照组使用传统人工面罩湿化吸氧,试验组运用Fisher&Paykel公司的HHFNC系统。观察两组患者医院获得性肺炎发生率、气道湿化效果、气道护理时间及并发症情况。结果:试验组湿化满意率高于对照组,医院获得性肺炎发生率,护理时间及并发症发生率均低于对照组,两组比较差异均有统计学意义(P<0.05)。结论:高流量湿化氧疗系统能为气管切开患者提供恒定的温度及湿度,减少并发症及护理时间,降低医院获得性肺炎的发生率。

목적:탐토고류량습화양료계통대기관절개환자의원획득성폐염적영향。방법:장75례균이탈리호흡궤기관절개환자안조수궤수자표법분위시험조화대조조,시험조45례,대조조30례。대조조사용전통인공면조습화흡양,시험조운용Fisher&Paykel공사적HHFNC계통。관찰량조환자의원획득성폐염발생솔、기도습화효과、기도호리시간급병발증정황。결과:시험조습화만의솔고우대조조,의원획득성폐염발생솔,호리시간급병발증발생솔균저우대조조,량조비교차이균유통계학의의(P<0.05)。결론:고류량습화양료계통능위기관절개환자제공항정적온도급습도,감소병발증급호리시간,강저의원획득성폐염적발생솔。
Objective:To investigate the preventive effect of the high-flow humidification oxygen therapy system on hospital-acquired pneumonia in tracheotomied patients.Method:A total of 75 patients had been out of breathing machine with tracheotomy were randomly divided into experimental group(45 cases) and control group(30 cases). The patients in the experimental group received heated and humidified high-flow oxygen therapy with the new system,while the patients in the control group were managed with traditional artificial airway humidification. The incidence of hospital acquired pneumonia,Airway of wetting effect, Airway nursing time and the complications were observed in both groups.Result:Airway moist satisfaction in experimental group was more high than control group, the incidences of hospital acquired pneumonia and complications in the experimental group were lower than that of control group and the airway nursing time each day for experimental group was shorter than that of control group simultaneously,and the difference between the two groups was statistically significant(P<0.05). Conclusion:The new type of high-flow humidification oxygen therapy system can improve comfort and efficacy of the oxygen therapy in tracheotomied patients, reduce complications and nursing time,reduce the incidence rate of hospital acquired pneumonia.
