目的:观察微波联合光动力疗法治疗男性尿道口尖锐湿疣的疗效和不良反应。方法将76例男性尿道口尖锐湿疣患者分为实验组和对照组,实验组采用微波联合光动力治疗,先用微波灼烧较大的疣体组织,再将5-氨基酮戊酸采用自制管状敷药器涂抹在病灶处,保留3 h后采用635 nm红光照射20~30 min,能量密度100 J/cm2;对照组仅采用微波治疗。每周治疗1次,共3周。结果实验组患者经过3次治疗后,治愈率90.6%,复发率9.4%,不良反应主要为照射时尿道灼痛和刺激,术后尿道分泌物增多。对照组患者经过3次微波治疗后,治愈率为39.1%,复发率高达60.9%,不良反应较严重,部分患者出现瘢痕。结论微波联合光动力疗法治疗男性尿道口尖锐湿疣,治愈率高达90.6%,不良反应小。

목적:관찰미파연합광동력요법치료남성뇨도구첨예습우적료효화불량반응。방법장76례남성뇨도구첨예습우환자분위실험조화대조조,실험조채용미파연합광동력치료,선용미파작소교대적우체조직,재장5-안기동무산채용자제관상부약기도말재병조처,보류3 h후채용635 nm홍광조사20~30 min,능량밀도100 J/cm2;대조조부채용미파치료。매주치료1차,공3주。결과실험조환자경과3차치료후,치유솔90.6%,복발솔9.4%,불량반응주요위조사시뇨도작통화자격,술후뇨도분비물증다。대조조환자경과3차미파치료후,치유솔위39.1%,복발솔고체60.9%,불량반응교엄중,부분환자출현반흔。결론미파연합광동력요법치료남성뇨도구첨예습우,치유솔고체90.6%,불량반응소。
Objective To observe the efifcacy and safety of topical application of 5-ALA photodynamic therapy (PDT) in the treatment of condyloma acuminata in male urethral oriifce. Methods Seventy-six male patients with condyloma acuminata in urethral oriifce were devided into the experimental group and the control group according to the patient’s will and economic status. Fifty-three patients in the experimental group were treated with microwave burning and photodynamic therapy, once per week, total for 3 weeks, while the other 23 patients in the control goup were treated with microwave burning only. The cases were followed up at regular visits for three months. Results For the experimental group, the cure rate was 90.6%, and the recurrence rate was about 9.4%. The common side effects were pain, burning-like sensation and increase of urethral secretion. For the control group, the cure rate was 39.1%and the recurrence rate was up to 60.9%(P<0.01). The control group showed side effects as pain and mild scar in the urethral. Conclusion PDT plus microwave burning is an efifcient way to treat condyloma acuminata in urethral oriifce, with high cure rate and better acceptance.
