目的:分别根据血β2-微球蛋白(β2-MG)、24 h尿蛋白,血浆纤维蛋白原(FG)和D-二聚体(DD)临床检测值,初步评估结节性血管炎患者肾损害和纤溶-凝血系统异常,以及两种异常的相关性,以利于临床上对结节性血管炎患者肾损害和纤溶-凝血系统异常进行早期干预。方法回顾性分析2011年5月-2013年9月34例结节性血管炎患者的血β2-MG、24 h尿蛋白、FG和DD的临床检测值,分别评估结节性血管炎肾损害和纤溶-凝血系异常及其临床意义。结果血β2-MG和24 h尿蛋白同时升高的患者占11.76%,FG和DD同时升高的患者占29.41%,血β2-MG和FG同时升高的患者占11.76%。采用Pearson积矩相关分析显示血β2-MG与FG两变量直线相关系数r=0.7335,P<0.005,血β2-MG与FG两变量之间存在正相关。结论少部分结节性血管炎患者有肾损害和纤溶-凝血系统异常,且肾损害和纤溶-凝血系统异常具有很高的正相关性,两种异常可能互为危险因素。

Objective Through the analysis of the results of the clinical tests including serumβ2-micro-globulin (β2-MG), 24 hour urinary protein, plasma ifbrinogen(FG) and two D-dimer(DD), preliminarily assess the kidney damage and the anomalies of ifbrinolytic and coagulative system and their correlations in the hospitalized patients with vasculitis nodular. The results will provide assistance for the clinician to give an early intervention to the hospitalized patients with erysipelas who suffering from renal damage and the anomalies of ifbrinolytic and coagulative system. Methods Thirty four hospitalized patients with nodular vasculitis from May 2011 to September 2013 were collected and retrospectively analyzed. Laboratory data including serum β2-MG, 24 hour urinary protein, FG and DD of the patients were analyzed, so as to assess the patients’ renal damage and the anomalies of ifbrinolytic and coagulative system. Results Both serum β2-MG and 24 hour urinary protein value were increased in 11.76% of the patients; both FG and DD’ s values were increased in 29.41%of the patients;both Serumβ2-MG and FG were increased in 11.76%of the patients. The result of Pearson product-moment correlation analysis showed that there was a positive correlation between serum β2-MG and FG (variable linear correlation coefifcient r=0.7335, P<0.005). Conclusion A minority of the hospitalized patients with nodular vasculitis were complicated with renal damage and anomalies of ifbrinolytic and coagulative system. Renal damage and the anomalies of ifbrinolytic and coagulative system may affect each other, meanwhile as the risk factors to each other, there is a high positive correlation between the two factors.
