目的：分析肿瘤专科医院住院患者不合理用药情况。方法收集2012年1月~2013年12月经本院临床药师分层抽样点评审核的住院病历327份、1773条处方医嘱，对医嘱不合理用药情况进行统计分析。结果共汇总126份病历、211条医嘱存在不合理用药情况，表现为以下类型：药物联用不合理47例、超常规用药33例、抗菌素使用不合理51例、中药注射剂使用不合理80例。结论肿瘤专科医院住院患者不合理用药的表现复杂、形式多样，临床药师应提高专业技术知识，积极协助临床查房，以降低临床不合理用药及不良反应的发生率，保障患者安全用药。
목적：분석종류전과의원주원환자불합리용약정황。방법수집2012년1월~2013년12월경본원림상약사분층추양점평심핵적주원병력327빈、1773조처방의촉，대의촉불합리용약정황진행통계분석。결과공회총126빈병력、211조의촉존재불합리용약정황，표현위이하류형：약물련용불합리47례、초상규용약33례、항균소사용불합리51례、중약주사제사용불합리80례。결론종류전과의원주원환자불합리용약적표현복잡、형식다양，림상약사응제고전업기술지식，적겁협조림상사방，이강저림상불합리용약급불량반응적발생솔，보장환자안전용약。
Objective To analyze the situation of irrational drug use in inpatient of cancer Hospital. Methods 327 medical records of inpatient and 1773 doctor’advice in our hoapital from January 2012 to December 2013 were collect-ed and reviewed,audited with stratified sampling by clinical pharmacist.The situation of irrational drug use was statisti-cally analyzed. Results After summarized, 126 medical records of inpatient and 211 doctor’advice were unqualified,and showed the following types,the unreasonable combination of drugs were found in 47 cases,33 cases existed unconven-tional treatment,51 cases had inappropriate usage of antibiotics,and 80 cases happened unreasonable usage of tradition-al Chinese medicine injections. Conclusion The performance of irrational drug use in inpatient of Cancer Hospital is complicated and has different forms.To reduce the incidence of clinical adverse reactions and irrational drug use,and to protect medication safety for patients,clinical pharmacists should improve technical expertise,actively assist in clinical rounds.
