目的：观察不同浓度舒芬太尼配伍罗哌卡因在剖宫产术后镇痛中的临床效果。方法将100例行剖宫产术患者随机分为A、B、C、D组，在0.15%罗哌卡因的基础上分别给予0.3、0.4、0.5、0.6μg/ml舒芬太尼进行硬膜外镇痛，观察各组患者术后4、8、12、24、48 h的VAS评分、Bromage评分以及不良反应发生情况。结果 A组患者的镇痛效果欠佳，术后4、8、12、24、48 h的VAS评分高于其他3组，差异有统计学意义(P<0.05)，B、C、D组间差异无统计学意义(P>0.05)。 D组有2例发生运动阻滞，术后4、8、12、24、48 h的Bromage评分高于其他3组，差异有统计学意义(P<0.05)。 D组患者的不良反应发生率明显高于其他3组(P<0.05)。结论0.15%罗哌卡因联合0.5μg/ml舒芬太尼在剖宫产术后镇痛中的效果较好，不良反应发生率低。
목적：관찰불동농도서분태니배오라고잡인재부궁산술후진통중적림상효과。방법장100례행부궁산술환자수궤분위A、B、C、D조，재0.15%라고잡인적기출상분별급여0.3、0.4、0.5、0.6μg/ml서분태니진행경막외진통，관찰각조환자술후4、8、12、24、48 h적VAS평분、Bromage평분이급불량반응발생정황。결과 A조환자적진통효과흠가，술후4、8、12、24、48 h적VAS평분고우기타3조，차이유통계학의의(P<0.05)，B、C、D조간차이무통계학의의(P>0.05)。 D조유2례발생운동조체，술후4、8、12、24、48 h적Bromage평분고우기타3조，차이유통계학의의(P<0.05)。 D조환자적불량반응발생솔명현고우기타3조(P<0.05)。결론0.15%라고잡인연합0.5μg/ml서분태니재부궁산술후진통중적효과교호，불량반응발생솔저。
Objective To observe the clinical effect of different concentrations recipe of sufentanil combined with ropi-vacaine for the analgesia after cesarean section. Methods 100 patients with cesarean section were randomly divided into A,B,C and D group.The patients were given 0.15% ropivacaine combined with 0.3,0.4,0.5,0.6 μg/ml sufentanil for epeidural analgesia respectively.The VAS score,Bromage score and side effects were recorded after operation 4,8,12,24 and 48 hours respectively. Results The VAS score of group A after operation 4,8,12,24 and 48 hours were significantly higher than other three groups,the difference was statistically significant (P<0.05).There was no significant difference of the VAS score between groups B,C and D (P>0.05).2 cases of motor block were generated in group D.The Bromage score in group D after operation 4,8,12,24 and 48 hours were higher than other three groups,the difference was statisti-cally significant (P<0.05).Incidence adverse reactions of group D were significantly higher than other three groups (P<0.05). Conclusion 0.5 μg/ml sufentanil combined with 0.15% ropivacaine is effective and safe in analgesia after ce-sarean section and has lower incidence of the adverse effect.
