目的：探讨腹腔镜联合切除术治疗胆囊良性疾病合并阑尾炎的临床效果。方法选取本院2011年3月~2013年3月收治的64例胆囊良性疾病合并阑尾炎患者，将其随机分为对照组和研究组各32例，对照组实施开腹阑尾术，研究组实施腹腔镜联合切除术，比较两组的手术时间、住院时间、并发症发生率及患者对疗效的满意度。结果研究组平均手术时间为(75.5±10.6)min，平均住院天数为(8.7±2.1)d。无中转开腹，随访5~18个月，无一例发生阑尾残端瘘、胆瘘、粘连肠梗阻等并发症，1例并发切口感染，并发症发生率为3.1%(1/32)，对治疗效果的满意度高达96.9%。对照组平均手术时间为(102.3±21.7)min，平均住院天数为(15.8±3.8) d，并发症发生率为15.6%，疗效满意度为84.4%。两组比较，差异有统计学意义(P<0.05)。结论腹腔镜联合胆囊切除术治疗胆囊良性疾病合并阑尾炎患者，创伤小，安全系数高，患者疼痛感轻，并发症少，值得临床推广应用。

목적：탐토복강경연합절제술치료담낭량성질병합병란미염적림상효과。방법선취본원2011년3월~2013년3월수치적64례담낭량성질병합병란미염환자，장기수궤분위대조조화연구조각32례，대조조실시개복란미술，연구조실시복강경연합절제술，비교량조적수술시간、주원시간、병발증발생솔급환자대료효적만의도。결과연구조평균수술시간위(75.5±10.6)min，평균주원천수위(8.7±2.1)d。무중전개복，수방5~18개월，무일례발생란미잔단루、담루、점련장경조등병발증，1례병발절구감염，병발증발생솔위3.1%(1/32)，대치료효과적만의도고체96.9%。대조조평균수술시간위(102.3±21.7)min，평균주원천수위(15.8±3.8) d，병발증발생솔위15.6%，료효만의도위84.4%。량조비교，차이유통계학의의(P<0.05)。결론복강경연합담낭절제술치료담낭량성질병합병란미염환자，창상소，안전계수고，환자동통감경，병발증소，치득림상추엄응용。
Objective To explore the effects of laparoscopy combined with resection in the treatment of benign gall-bladder diseases complicated with appendicitis. Methods 64 patients with benign gallbladder diseases and appendicitis in our hospital from March 2011 to March 2013 were selected and randomly divided into control group and research group,32 patients in each group,laparotomy was given to the control group,laparoscopy combined with resection was giv-en to the research group.The surgery time,hospitalization time,complications and patient satisfaction of two groups were compared. Results In research group,the average surgery time was (75.5±10.6) min,the average hospitalization time was (8.7±2.1) d.Based on the follow-up of 5-18 months,no patients were converted to laparotomy and no patients showed appendiceal nub fistula,biliary fistula,adhesive intestinal obstruction and other complications,only 1 patient showed complicated incision infection,the incidence of complications was 3.1% (1/32),and the satisfaction rate was 96.9%;in control group,the average surgery time was (102.3±21.7) min,average hospitalization time was (15.8±3.8) d,incidence of complications was 15.6% and satisfaction rate was 84.4%;the differences were significant (P<0.05). Conclusion La-paroscopy combined with resection in the treatment of benign gallbladder diseases complicated with appendicitis has minor trauma,high safety,less pain and less complications,which is worthy of clinical promotion and application.
