目的:探讨无创通气治疗慢性阻塞性肺疾病(COPD)合并重症呼吸衰竭的临床效果。方法抽取2011年12月~2013年12月在本院治疗的COPD急性发作合并呼吸衰竭患者72例,随机分为观察组与对照组,每组36例。对照组采用吸氧、药物治疗等常规对症支持治疗,观察组在对照组的基础上给予无创正压通气治疗,比较两组患者的血气分析结果、不良发应发生率。结果治疗3 d后,两组患者的pH、PaO2、PaCO2等均明显改善,观察组的改善情况明显优于对照组(P<0.05);观察组的不良反应发生率为13.8%,对照组为11.1%,两组比较,差异无统计学意义(P>0.05)。结论无创通气治疗COPD合并重症呼吸衰竭能快速缓解患者的临床症状,提高PaO2,降低PaCO2,提高患者的生存质量,值得临床推广。

목적:탐토무창통기치료만성조새성폐질병(COPD)합병중증호흡쇠갈적림상효과。방법추취2011년12월~2013년12월재본원치료적COPD급성발작합병호흡쇠갈환자72례,수궤분위관찰조여대조조,매조36례。대조조채용흡양、약물치료등상규대증지지치료,관찰조재대조조적기출상급여무창정압통기치료,비교량조환자적혈기분석결과、불량발응발생솔。결과치료3 d후,량조환자적pH、PaO2、PaCO2등균명현개선,관찰조적개선정황명현우우대조조(P<0.05);관찰조적불량반응발생솔위13.8%,대조조위11.1%,량조비교,차이무통계학의의(P>0.05)。결론무창통기치료COPD합병중증호흡쇠갈능쾌속완해환자적림상증상,제고PaO2,강저PaCO2,제고환자적생존질량,치득림상추엄。
Objective To investigate the clinical efficacy of non-invasive ventilation in the treatment of chronic ob-structive pulmonary disease (COPD) combined with severe respiratory failure. Methods Seventy-two patients with COPD acute exacerbation combined with respiratory failure treated in our hospital from December 2011 to December 2013 were selected and randomly divided into the observation group and the control group,with 36 patients in each group.The con-trol group received conventional symptomatic and supportive treatment,such as oxygen uptake and medication.The ob-servation group received non-invasive ventilation on the basis of the control group.The blood gas analysis results,the adverse reaction of the two groups were compared. Results After 3 days treatment,pH,PaO2 and PaCO2 were improved significantly in the two groups,the observation group had significantly better improvement situation than the control group (P<0.05);the incidence rate of adverse reactions of the observation group was 13.8%and that of the control group was 11.1%,with no statistically difference between the two groups (P>0.05). Conclusion Non-invasive ventilation treat-ing COPD combined with severe respiratory failure can relieve the clinical symptoms of patients rapidly,improve PaO2, lower PaCO2 and improve the quality of life of patients,which is worthy of clinical promotion.
