目的:研究和分析无创呼吸机治疗COPD急性加重期合并II型呼衰的疗效。方法选取我院2011年10月-2013年10月收治的60例COPD患者临床资料进行分析研究,对比治疗前后患者的对动脉血的PH、PO2、PCO2等的变化。结果治疗组经无创呼吸机治疗前pH、PO2、PCO2值分别为(7.28±0.05)、(50.2±8.9)、(72.1±12.1),治疗后各值分别为(7.35±0.04)、(70.2±9.1)、(58.6±9.6);对照组经无创呼吸机治疗前pH、PO2、PCO2值分别为(7.28±0.04)、(51.1±8.6)、(70.4±11.1),治疗后各值为(7.29±0.05)、(62.2±9.2)、(68.9±9.8),治疗后患者的pH、PO2升高,PCO2下降,与对照组比较差异有显著性(P<0.05)。结论早期使用无创呼吸机治疗COPD急性加重期合并II型呼衰疗效较好,并发症少。

Objective To study and analyze the curative effect of non-invasive ventilation in the treatment of acute exacerbation of COPD with type II respiratory failure. Methods The clinical data of 60 cases of COPD patients in our hospital from 2011 October to 2013 October were selected to conduct the analysis research, compare the change before and after the treatment in the arterial blood PH, PO2, PCO2 etc.. Results The treatment group by non-invasive ventilation in the treatment of PH, PO2, PCO2 values were (7.28±0.05), (50.2±8.9), (72.1±12.1), after treatment, the values were (7.35±0.04), (70.2±9.1), (58.6±9.6);the control group by nonin-vasive ventilator before treatment, PO2 PH, PCO2 values were (7.28±0.04), (51.1±8.6), (70.4 ±11.1), after treatment the value of (7.29±0.05), (62.2±9.2),(68.9±9.8), after treatment of patients with PH, PO2 increased, PCO2 decreased, there was significant difference com-pared with the control group (P<0.05). Conclusion The early use of non-invasive ventilation in treatment of acute exacerbation of COPD with type II respiratory failure better treatment effect, less complications.
