目的:综合评价信息传导与转录激活因子-4基因(STAT4基因)rs7574865位点单核苷酸多态性与类风湿关节炎易感性的相关性。方法:计算机检索Pubmed、Embase、中国生物医学文献数据库和万方数据库数据库等,并手工检索相关杂志,收集有关不同人群STAT4基因rs7574865位点单核苷酸多态性的等位基因频率、基因型频率与类风湿性关节炎易感性相关性的研究,采用Stata 12.0软件进行Meta分析;选择的遗传模型包括GT+TT与GG比较,TT与GT+GG比较,TT与GG比较以及等位基因T与G之间比较。结果:STAT4基因rs7574865位点T等位基因频率与类风湿性关节炎易感性可能具有相关性(OR=1.259,95%CI=1.202-1.319, P<0.001);T等位基因频率与类风湿性关节炎易感性的相关性在欧洲、亚洲、非洲、拉丁美洲人群中有统计学意义;在总体人口中STAT4基因rs7574865位点的基因型与类风湿性关节炎易感性可能具有相关性;在总体人口中STAT4基因rs7574865位点多态性与抗CCP抗体阳性或RF因子阳性类风湿性关节炎易感性仍可能具有相关性。结论:STAT4基因rs7574865位点单核苷酸多态性与类风湿性关节炎易感性可能具有相关性,而且这种相关性可能独立于患者RF因子以及抗CCP抗体水平。

Objective Many genetic studies have suggested an association of rs7574865 polymorphism of STAT4 gene and Rheumatoid arthritis (RA), but evidence remains conflicting. To investigate whether combined evidence to show the association between polymorphism in signal transducers and activators of transcription 4 (STAT4) gene rs7574865 polymorphism and RA. Methods A systematic search of all relevant studies pub-lished through Mar 2013 was conducted using the MEDLINE, EMBASE, OVID, and ScienceDirect. Seventeen case-control studies with a total of 28 comparisons (25300 RA patients and 26326 controls) met the inclusion criteria. Meta-analysis was conducted for genotype TT versus GT+GG (recessive model), GT+TT versus GG (dom-inant model), TT versus GG (codominant model) and T-allele. Results Meta-analysis showed an association between RA and the STAT4 rs7574865 TT genotype (OR = 1.430, 95%CI = 1.299-1.574, P<0.001), GT+TT genotype (OR = 1.352, 95%CI = 1.243-1.471, P<0.001) and T-allele (OR = 1.259, 95%CI = 1.202-1.319, P<0.001) in all subjects. Stratification of RA patients according to ethnic groups demonstrated that TT geno-type, GT+TT genotype and T-allele were found to be significantly associated with RA in Europeans, Asians, Afri-cans and Latin Americans. Subgroup analysis according to absence or presence of rheumatoid factor (RF)/anti-cy-clic citrullinated peptide (anti-CCP) positive or negative revealed that the association between rs7574865 poly-morphism of STAT4 gene and RA might be independent of the presence of RF and CCP antibodies. Conclusion This demonstrates that STAT4 rs7574865 polymorphism may confer susceptibility to RA. The association may not be dependent of the presence of RF and CCP antibodies. Because of the limited number of studies available in Africans and Latin Americans, further studies with large sample size are required.
