目的：探讨320排CT支持下经导管肝动脉化疗栓塞术（ TACE）治疗原发性肝癌（ PLC）的临床疗效。方法研究组45例患者术前先行320排CT检查，观察肿瘤影像表现及其供瘤动脉的起源走行和变异情况，并通过血管重建模型上的模拟训练，拟定插管及栓塞方案后行TACE；对照组45例经16排CT检查观察肿瘤影像表现后行TACE；分析比较两组患者的临床疗效和不良反应。结果研究组治疗有效率（68．9％）高于对照组（46．7％），而不良反应发生率低于对照组（P＜0．05）；研究组6个月、1年、2年生存率分别为97．8％、86．7％、57．8％，高于对照组的（88．9％、68．9％和40．0％）（P＜0．05）。结论320排CT可快速准确地评估PLC肿瘤及供血情况，在其支持下行TACE治疗可明显提高疗效及减轻不良反应。
목적：탐토320배CT지지하경도관간동맥화료전새술（ TACE）치료원발성간암（ PLC）적림상료효。방법연구조45례환자술전선행320배CT검사，관찰종류영상표현급기공류동맥적기원주행화변이정황，병통과혈관중건모형상적모의훈련，의정삽관급전새방안후행TACE；대조조45례경16배CT검사관찰종류영상표현후행TACE；분석비교량조환자적림상료효화불량반응。결과연구조치료유효솔（68．9％）고우대조조（46．7％），이불량반응발생솔저우대조조（P＜0．05）；연구조6개월、1년、2년생존솔분별위97．8％、86．7％、57．8％，고우대조조적（88．9％、68．9％화40．0％）（P＜0．05）。결론320배CT가쾌속준학지평고PLC종류급공혈정황，재기지지하행TACE치료가명현제고료효급감경불량반응。
Objective To explore the clinical efficacy of transcatherter arterial chemoembolization ( TACE) for primary liver cancer in the support of 320 rows CT.Methods 45 primary liver cancer patients in the study group received 320 rows CT examination before the operation , and tumor image presentation and the origin , course and variation of the tumor feeding artery were observed;through the simulation training of vascular reconstruction model ,TACE was performed after working out intubation and embolization scheme .45 patients in the control group received 16 rows CT examination and TACE was performed after observ-ing the tumor image presentation .Clinical efficacy and adverse reactions of the 2 groups were analyzed and compared .Results Effective rate of the study group was 68.0%,which was higher than that of the control group (46.7%).Occurrence rate of ad-verse reactions of the study group was lower than that of the control group (P<0.05).6-month,1-year and 2-year survival rates of the study group group were 97.8%,86.7%and 57.8%,which were higher than those of the control group (88.9%,68.9%and 40.0%)(P<0.05).Conclusion 320 rows CT can quickly and accurately evaluate tumor and blood supply situation of primary liver cancer ,and TACE can obviously increase curative effects and decrease adverse reactions with its support .
