目的：探讨宫颈上皮瘤样病变患者（ CIN）经过LEEP锥切术后HPV感染的变化情况。方法将309例经阴道镜下活检确诊为CIN患者，按照组织病理学诊断分为CINⅠ组、CINⅡ组、CINⅢ组，分别观察三组患者LEEP术前与术后HPV感染的差异及术后3、6、9、12个月HPV感染的差异。结果三组患者术前镜检与术后标本检查HPV感染情况基本一致，三组组内比较差异无统计学意义（P＞0．05），CINⅢ组HPV感染阳性率明显高于CINⅡ、CINⅠ组，三组间比较差异均具有统计学意义（P＜0．05）；三组HPV感染率术后3、6、9、12个月与术前比较均明显降低，且差异具有统计学意义（P＜0．05），三组术后3、6、9、12个月组间比较差异具有统计学意义（P＜0．05），CINⅠ组术后HPV残存率明显低于CINⅡ组和CINⅢ组，CINⅡ组明显低于CINⅢ组；三组术后12个月转阴率分别为CINⅠ（98．94％）、CINⅡ（92．95％）、CINⅢ（86．44％），三组之间两两比较差异具有统计学意义（P＜0．05）。结论 CIN病理级别越低HPV感染率越低，术后HPV残存率和复发率均明显低于CIN病理级别高者。
목적：탐토궁경상피류양병변환자（ CIN）경과LEEP추절술후HPV감염적변화정황。방법장309례경음도경하활검학진위CIN환자，안조조직병이학진단분위CINⅠ조、CINⅡ조、CINⅢ조，분별관찰삼조환자LEEP술전여술후HPV감염적차이급술후3、6、9、12개월HPV감염적차이。결과삼조환자술전경검여술후표본검사HPV감염정황기본일치，삼조조내비교차이무통계학의의（P＞0．05），CINⅢ조HPV감염양성솔명현고우CINⅡ、CINⅠ조，삼조간비교차이균구유통계학의의（P＜0．05）；삼조HPV감염솔술후3、6、9、12개월여술전비교균명현강저，차차이구유통계학의의（P＜0．05），삼조술후3、6、9、12개월조간비교차이구유통계학의의（P＜0．05），CINⅠ조술후HPV잔존솔명현저우CINⅡ조화CINⅢ조，CINⅡ조명현저우CINⅢ조；삼조술후12개월전음솔분별위CINⅠ（98．94％）、CINⅡ（92．95％）、CINⅢ（86．44％），삼조지간량량비교차이구유통계학의의（P＜0．05）。결론 CIN병리급별월저HPV감염솔월저，술후HPV잔존솔화복발솔균명현저우CIN병리급별고자。
Objective To study the changes of HPV infection in cervical intraepithelial neoplasia ( CIN) patients after LEEP conization.Methods 309 CIN patients confirmed by colposcopy biopsy were divided into group CIN Ⅰ,CINⅡand CINⅢ,difference of infection before and after LEEP and 3,6,9,12 months after surgery of the 3 groups were observed .Results HPV infection preoperative microscopic examination and postoperative results of the 3 groups were basically the same ,the differ-ences of the 3 groups were not statistically significant (P>0.05).HPV positive infection rate of group CINⅢ was significantly higher than those of group CIN Ⅰand CINⅡ,the difference of the 3 groups was statistically significant (P<0.05);3,6,9,12 months postoperative HPV infection rates of the 3 groups were significantly decreased ,and the difference was statistically signifi-cant(P<0.05),there had statistical difference in 3,6,9,12 months postoperative HPV infection rates of the 3 groups (P<0.05),postoperative HPV residual rate of group CIN Ⅰwas significantly lower than those of group CINⅡand CINⅢ,postopera-tive HPV residual rate of group CINⅡwas significantly lower than that of the CIN 3 group.Postoperative 12 months negative-inve-rte rates of the 3 groups were CIN Ⅰ(98.94%),CINⅡ(92.95%) and CINⅢ(86.44%),there were statistical difference be-tween the 3 groups(P<0.05).Conclusion Lower the CIN staging,the lower is the infection rate of HPV,HPV postoperative survival rates and recurrence rates in patients with low CIN stage are significantly lower than those with high CIN stage .
