目的:探讨在男性2型糖尿病患者中胰岛素抵抗与前列腺特异性抗原( prostate specific antigen,PSA)的相关性。方法男性2型糖尿病183例,检测空腹血糖( FPG)、空腹胰岛素( FINS)、糖化血红蛋白( HbA1C)、血脂系列( TC、TG、LDL、HDL)、游离前列腺特异性抗原( fPSA)和总前列腺特异性抗原( tPSA)浓度等相关临床指标,计算胰岛素抵抗指数( HOMA-IR)、体质指数(BMI)。根据HOMA-IR分为胰岛素抵抗组(IR≥2.8)和非胰岛素抵抗组(IR<2.8);根据tPSA的中位数0.732μg/L,将患者分为tPSA≥0.732μg/L组和tPSA<0.732μg/L组。结果胰岛素抵抗组(69例)患者的BMI明显高于非胰岛素抵抗组(114例)(P<0.05),tPSA明显低于非胰岛素抵抗组(P<0.05)。 tPSA≥0.732μg/L组(92例)的BMI、HOMA-IR均低于tPSA<0.732μg/L组(91例)(均P<0.05)。 tPSA与年龄、fPSA、LDL呈正相关( r分别为0.345、0.865、0.200;均P<0.05),与IR、BMI呈负相关(r分别为-0.151、-0.155;均P<0.05)。结论胰岛素抵抗和肥胖是导致PSA下降的因素。

목적:탐토재남성2형당뇨병환자중이도소저항여전렬선특이성항원( prostate specific antigen,PSA)적상관성。방법남성2형당뇨병183례,검측공복혈당( FPG)、공복이도소( FINS)、당화혈홍단백( HbA1C)、혈지계렬( TC、TG、LDL、HDL)、유리전렬선특이성항원( fPSA)화총전렬선특이성항원( tPSA)농도등상관림상지표,계산이도소저항지수( HOMA-IR)、체질지수(BMI)。근거HOMA-IR분위이도소저항조(IR≥2.8)화비이도소저항조(IR<2.8);근거tPSA적중위수0.732μg/L,장환자분위tPSA≥0.732μg/L조화tPSA<0.732μg/L조。결과이도소저항조(69례)환자적BMI명현고우비이도소저항조(114례)(P<0.05),tPSA명현저우비이도소저항조(P<0.05)。 tPSA≥0.732μg/L조(92례)적BMI、HOMA-IR균저우tPSA<0.732μg/L조(91례)(균P<0.05)。 tPSA여년령、fPSA、LDL정정상관( r분별위0.345、0.865、0.200;균P<0.05),여IR、BMI정부상관(r분별위-0.151、-0.155;균P<0.05)。결론이도소저항화비반시도치PSA하강적인소。
Objective To evaluate the relationship between insulin resistance and serum prostat specific antigen( PSA) levels in male patients with type 2 diabetics.Methods Totally 183 male patients with type 2 diabetics were included in this study.Biochemi-cal analyses including fasting plasma glucose(FPG),serum fasting levels of insulin(FINS),HbA1C,lipid,free prostate specific antigen ( fPSA) and total prostate specific antigen ( tPSA) were performed.Insulin resistance index( HOMA-IR) and body mass index ( BMI) were calculated.Based on HOMA-IR,patients were divided into insulin resistant group (IR>2.8) and non-insulin resistant group (IR<2.8) .Based on median of tPSA,patients were divided into group of tPSA≥0.732μg/L and group of tPSA<0.732μg/L.Results BMI was more increased in insulin resistant group (69 cases) compared with non-insulin resistant group (114 cases) (P<0.05).tP-SA was more increased in non-insulin resistant group compared with insulin resistant group(P<0.05).BMI and HOMA-IR were more increased in group of tPSA<0.732 μg/L (91 cases) compared with group of tPSA≥0.732μg/L (92 cases) (both P<0.05).tPSA was positively correlated with age,fPSA and LDL (r=0.345,0.865,0.200;all P<0.05),and negatively correlated with IR and BMI (r=-0.151,-0.155;both P<0.05).Conclusion Insulin resistance and obesity play roles in the PSA decreasing.
