目的:评价64层螺旋CT血管造影(MSCTA)及血管生成素-2(Ang-2)表达在胰腺癌胰周血管侵犯中的作用。方法术前应用MSCTA对22例胰腺癌胰周主要血管进行CTA三维成像,并与手术的所见进行比较;通过免疫组化染色法检测22例胰腺癌患者癌组织及癌旁组织(癌周血管附近组织)中Ang-2的表达。结果22例胰腺癌患者8段共176支血管中,MSCTA术前诊断41支血管受侵犯,135支血管未受侵犯,术中发现40支血管受侵犯,131支血管未受侵犯。胰腺癌及癌旁组织中Ang-2阳性表达率分别为46.8%和36.5%, CT提示有胰周血管侵犯的组织Ang-2阳性96.5%,CT提示胰周血管未侵犯组织Ang-2阳性率16.6%。结论 MSCTA结合胰腺癌胰周血管组织Ang-2的表达可提高对胰腺癌胰周血管受侵的准确率。

목적:평개64층라선CT혈관조영(MSCTA)급혈관생성소-2(Ang-2)표체재이선암이주혈관침범중적작용。방법술전응용MSCTA대22례이선암이주주요혈관진행CTA삼유성상,병여수술적소견진행비교;통과면역조화염색법검측22례이선암환자암조직급암방조직(암주혈관부근조직)중Ang-2적표체。결과22례이선암환자8단공176지혈관중,MSCTA술전진단41지혈관수침범,135지혈관미수침범,술중발현40지혈관수침범,131지혈관미수침범。이선암급암방조직중Ang-2양성표체솔분별위46.8%화36.5%, CT제시유이주혈관침범적조직Ang-2양성96.5%,CT제시이주혈관미침범조직Ang-2양성솔16.6%。결론 MSCTA결합이선암이주혈관조직Ang-2적표체가제고대이선암이주혈관수침적준학솔。
Objective To evaluate the effect of the 64-slice CT angiography examination with the level of Angiopoietin-2 on the peripancreatic vessels invasion of pancreatic carcinoma. Methods 22 Patients with pancreatic carcinoma were scanned by MSCTA, the state of the peripancreatic vessel of encroached by pancreatic carcinoma were evaluated in three-dimension. Correlation was made between MSCTA findings and surgical results. Immuno-histochemical technique was used to detect the expression of Ang-2 in 22 cases of Pancreatic Carcinoma and para-cancer tissue. Results Among the total 176 vessels of 22 patients underwent operation were involved, 41 vessels were invasive and 135 vessels were non-invasive affirmed with CTA. 40vessels were invasive and 136 vessels were non-invasive were affirmed in the operation. The positive expression of Ang-2 in pancreatic cancer and para-cancer tissue were 46.8%, 36.5%. The positive expression of Ang-2 in peripancreatic vessels invasion affirmed with CTA were 96.5%,while The positive expression of Ang-2 in peripancreatic vessels non-invasion affirmed with CTA were 16.6%. Conclusion Ang-2 and MSCTA play a syndetic role in evaluate the peripancreatic vessels invasion of pancreatic cancer.
