目的：通过观察磁共振灌注成像(PWI)反映的组织微血管分布情况，鉴别经过手术的脑胶质瘤患者放疗后的复发与放射性损伤。方法选取60例临床确诊为脑胶质瘤且放疗后的患者，回顾性分析其PWI图像及手术、随访结果，并作统计分析。结果60例患者最终经手术和随访证实30例复发，12例放射性脑损伤，复发组的相对脑血容量(rCBV)、相对脑血流量(rCBF)比值明显高于放射性损伤组比值，差异有统计学意义(P<0.05)。结论 PWI技术对鉴别脑胶质瘤放疗后期复发与放射性损伤有重要的参考价值。
목적：통과관찰자공진관주성상(PWI)반영적조직미혈관분포정황，감별경과수술적뇌효질류환자방료후적복발여방사성손상。방법선취60례림상학진위뇌효질류차방료후적환자，회고성분석기PWI도상급수술、수방결과，병작통계분석。결과60례환자최종경수술화수방증실30례복발，12례방사성뇌손상，복발조적상대뇌혈용량(rCBV)、상대뇌혈류량(rCBF)비치명현고우방사성손상조비치，차이유통계학의의(P<0.05)。결론 PWI기술대감별뇌효질류방료후기복발여방사성손상유중요적삼고개치。
Objective To identify tumor recurrence or radiation-induced brain injury of the patients with glioma after operation and radiotherapy, by using Magnetic Resonance Perfusion Weighted Imaging (PWI) which can display the organization of microvascular distribution and blood flow perfusion. Materials and Methods 60 patients with glioma after operation and radiotherapy were selected. Their PWI imaging, operation and follow-up results were retrospectively analyzed. Results Among 60 patients, 30 patients were confirmed recurrence by the operation and follow-up, 12 patients were radiation-induced brain injury. The relative cerebral blood volume (rCBV), relative cerebral blood flow(rCBF)of recurrence group obviously higher than the radiation-induced brain injury group, the differences had statistical significance (P<0.05). Conclusion The PWI technique has an important value on identifying tumor recurrence and radiation-induced brain injury.
