目的：对超声导向经会阴穿刺冷冻治疗中晚期前列腺癌的效果进行探讨分析。方法对我院22例中晚期前列腺癌患者实施经会阴穿刺液氮冷冻前列腺肿瘤组织，对比穿刺前后 PSA、前列腺及肿瘤体积的影像学结果。结果术后3个月患者的瘤体已经基本消失，仅残留少量的瘢痕组织。前列腺体积由术前的（64.3±4.8）ml降至（25.6±3.2）ml，PSA 术前为（39.1±10.7）ng/ ml，术后12个月、24个月时分别为（3.9±1.7）和（3.1±1.4）ng/ml。患者于术后6个月之前进行前列腺多点穿刺进行活检，2例患者为阳性。所有患者均未出现相关的冷冻并发症。结论超声导向经会阴穿刺冷冻治疗中晚期前列腺癌的效果比较理想，在临床上值得推广应用。
목적：대초성도향경회음천자냉동치료중만기전렬선암적효과진행탐토분석。방법대아원22례중만기전렬선암환자실시경회음천자액담냉동전렬선종류조직，대비천자전후 PSA、전렬선급종류체적적영상학결과。결과술후3개월환자적류체이경기본소실，부잔류소량적반흔조직。전렬선체적유술전적（64.3±4.8）ml강지（25.6±3.2）ml，PSA 술전위（39.1±10.7）ng/ ml，술후12개월、24개월시분별위（3.9±1.7）화（3.1±1.4）ng/ml。환자우술후6개월지전진행전렬선다점천자진행활검，2례환자위양성。소유환자균미출현상관적냉동병발증。결론초성도향경회음천자냉동치료중만기전렬선암적효과비교이상，재림상상치득추엄응용。
ObJective:To stuy the effect of ultrasound guided perineal puncture in the treatment of advanced prostate cancer. Methods:The advanced prostate cancer patients of 22 cases in our hospital had implementation of transperineal puncture cryotherapy prostate tumor tissue. PSA,puncture prostate and volume of tumor imaging results were compared. Results:The volume of prostate decreased from(64. 3 ± 4. 8)ml preoperative to(25. 6 ± 3. 2)ml. The preoperative value of PSA was(39. 1 ± 10. 7)ng/ ml,and droped to(3. 9 ± 1. 7)and(3. 1 ± 1. 4)ng/ ml,respectively in the 12,24 months after operation. Postoperative biopsies were positive only in two cases in the 6th months. Frozen related complications were not seen in all the patients. Conclusion:Ultrasound guided transperineal puncture in the treatment of advanced prostate cancer effect is relatively good,and is worthy of clinical application.
