目的:探讨乙型肝炎病毒( HBV )标志物定量检测指标与HBV DNA定量水平的相关性。方法从2013年1月-2014年6月检验科采用酶联免疫吸附试验( ELISA)确诊的在感染性疾病科门诊就诊和住院的大三阳368例和小三阳507例中,各随机选取50例作为大三阳组和小三阳组。大三阳组和小三阳组的血标本同时做HBV标志物定量和HBV DNA定量检测。结果大三阳组HBsAg含量和HBV DNA定量检测结果均明显高于小三阳组( P<0.01);大三阳组HBV DNA定量阳性率明显高于小三阳组(P<0.01);HBV DNA阳性的大三阳患者HBV DNA含量与HBsAg、HBeAg含量均具有相关性(P<0.01)。结论 HBV DNA水平与HBV标志物中的HBsAg和HbeAg水平呈明显正相关;定量检测HBV DNA、HBsAg和HbeAg对判断HBV感染者传染性具有重要临床意义。

Objective To investigate the correlation between quantitative detection index associated to Hepatitis b virus ( HBV) markers and the quantitative level of HBV DNA. Methods A total of 368 patients with great three posi-tive results and 507 patients with small three positive results were confirmed by using enzyme-linked immunosorbent assay ( ELISA) during January 2013 and June 2014 in infectious diseases department, and 50 patients were randomly selected from the 368 patients as great three positive results group ( group A) , and 50 patients were randomly selected from 507 patients as small three positive results group ( group B) . Blood samples of the two groups underwent quantitative detec-tions of HBV markers and HBV DNA simultaneously. Results HBsAg content and HBV DNA quantitative test results were significantly higher than those in group B (P<0. 01);positive rate of HBV DNA quantitation in group A was signif-icantly higher than that in group B (P<0. 01);HBV DNA content had correlation with the HBsAg and HBeAg contents in great three positive result patients with positive HBV DNA (P<0. 01). Conclusion HBV DNA level has obviously positive correlation with HBsAg and HbeAg levels, and quantitative detections of HBV DNA, HBsAg and HbeAg have important clinical significance for judging HBV infectivity.
